การบำบัดทางการพยาบาลในผู้ป่วยใส่ท่อช่วยหายใจและใช้เครื่องช่วยหายใจ : การทบทวนวรรณกรรมอย่างเป็นระบบ

บทคัดย่อ

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อ 1) จำแนกรูปแบบของการบำบัดทางการพยาบาล และ 2) วิเคราะห์ องค์ความรู้เกี่ยวกับการบำบัดทางการพยาบาลในผู้ป่วยใส่ท่อช่วยหายใจและใช้เครื่องช่วยหายใจ สืบค้น ข้อมูลจากฐานข้อมูลอิเลคทรอนิกส์ จากงานวิจัยที่ตีพิมพ์ในประเทศไทย ตั้งแต่ปี พ.ศ. 2528 ถึง พ.ศ. 2551 จำนวน 20 เรื่อง ผลการศึกษาพบว่า การบำบัดทางการพยาบาลในผู้ป่วยใส่ท่อช่วยหายใจ และใช้เครื่องช่วยหายใจ สามารถจำแนกโดยใช้แนวทางของ Nursing Intervention Classification (NIC) ได้ 4 รปแบบ ดังนี้ 1) การบำบัดทางด้านสรีรวิทยาพื้นฐาน ได้แก่ การนวดแผนไทยประยุกต์ 2) การบำบัดทางด้านสรีรวิทยาที่มีความซับซ้อน ได้แก่ การระบายเสมหะแบบประยุกต์ การหย่าเครื่องช่วยหายใจ และ การบำบัดด้วยการใช้ออกซิเจน 3) การบำบัดทางการพยาบาลด้านพฤติกรรม ได้แก่ การใช้ดนตรีบำบัด การสร้างรปแบบการสื่อสาร การให้ข้อมูลเพื่อเตรียมพร้อม และการฝึกสติให้อยู่กับปัจจุบัน และ 4) การบำบัดทางการพยาบาลเพื่อส่งเสริมความปลอดภัย ได้แก่ การป้องกันการติดเชื้อ

ผลการศึกษา แสดงถึงรูปแบบการพยาบาลที่ช่วยลดภาวะแทรกซ้อนและผลกระทบที่เกิดฃื้นแก่ผู้ป่วย สามารถนำไปปรับใช้ในการให้การดูแลผู้ป่วยที่ใส่ท่อช่วยหายใจ และใช้เครื่องช่วยหายใจที่รับการรักษาใน โรงพยาบาล เพื่อส่งเสริมให้ผู้ป่วยเกิดความปลอดภัย และเกิดความสุขสบายทั้งด้านร่างกายและจิตใจ ตั้งแต่ ก่อนการใช้เครื่องช่วยหายใจ ขณะใช้เครื่องช่วยหายใจ จนกระทั้งหย่าเครื่องช่วยหายใจ และสามารถหายใจได้เองในที่สุด

Abstract

The objectives of this study were to: 1) classify the forms of the nursing therapy, and 2) analyze the body of knowledge about the nursing therapy for patients who have been intubated with mechanical ventilators. The study was conducted by examining data from electronic database of the published researches in Thailand from 1985 to 2008. The obtained researches were 20 studies. It was found that the nursing therapy for patients who have been intubated and used mechanical ventilator could be classified based on the models of Nursing Intervention Classification (NIC) (2004) into 4 models: 1) the domain of physiological basic, i.e. modified Thai massage; 2) the domain of complex physiology, i.e. modified secretion drainage method, mechanical ventilation weaning and oxygen therapy; 3) the domain of behavior, i.e. music therapy, communication method, preparatory sensory information and here and now technique, and 4) the domain of safety, i.e. infection control.

The result of the study found that the nursing therapies which were used to reduce infection and side effects of the patients could be adapted and applied to use with patients who have been intubated with mechanical ventilators. It also helped to assure the patients to feel more physically and mentally safe before using the ventilators, during using the ventilators and weaning from the ventilators and finally the patients could breathe by themselves.
