The Development of the Case Management Model for Patients with Serious Mental Illness: Waeng District Case Study

Abstract
The research and development aimed to develop and test the case management model for psychiatric patients with serious mental illness (SMI) in the community. The research consisted of two phases: 1) Developed a case management model for psychiatric patients with serious mental illness (SMI) based on the conceptual framework of Powell and literature review of case management as well as content analysis. A case management guideline was then designed and the content validity verified by three experts, with IOC yielding a value of .81. and 2) Tried out the case management model with six participants, registered nurses. Qualitative data were analyzed by using content analysis, and quantitative data were analyzed by using descriptive statistics. Results: The results showed that the case management of patients with SMI for nurses in community consisted of; 1) the case management categories including registered nurses as a case manager, case management team in community, and clinical pathway; 2) the case management of patients with SMI guideline; and 3) the case management process through IMed@home application and testing the case management for 6 months that the patients symptoms no relapse and no hospitalization as well as overall mean score of nurse’s satisfaction for implementation was very good Conclusion: the case management model for patients with serious mental illness in community could be applied by registered nurses to improve care in the community for psychiatric patients with serious mental illness.
