บทคัดย่อ
วัตถุประสงค์: ศึกษาผลของโปรแกรมการดูแลผู้ป่วยติดเตียงที่บ้านต่อความสามารถและคุณภาพการดูแลของญาติผู้ดูแลผู้ป่วยติดเตียงในตำบลท่าอุเทน จังหวัดนครพนม วิธีการ: การวิจัยกึ่งทดลองแบบกลุ่มเดียว วัดผลก่อนและหลังการทดลอง กลุ่มตัวอย่าง คือ ญาติผู้ดูแลหลักของผู้ป่วยติดเตียง จำนวน 27 คน คัดเลือกตามคุณสมบัติที่กำหนด เข้าร่วมโปรแกรมการดูแลผู้ป่วยติดเตียงที่บ้าน 70 ชั่วโมงประกอบด้วย บรรยาย 24 ชั่วโมง สาธิตย้อนกลับ 32 ชั่วโมง และฝึกภาคสนาม 14 ชั่วโมง วัดผลลัพธ์โดยประเมินความสามารถในการดูแล แบ่งเป็น 3 ด้าน คือ ความรู้ ทัศนคติ และการปฏิบัติ และสังเกตคุณภาพการดูแลของญาติผู้ดูแลผู้ป่วยติดเตียง ก่อนและหลังการเข้าร่วมโปรแกรม ค่าดัชนีความตรงตามเนื้อหาของทั้ง 3 เครื่องมือ เท่ากับ 1 และค่าความเที่ยง เท่ากับ .71, .86, .93 และ .83 ตามลำดับ เก็บข้อมูลระหว่างเดือน สิงหาคม ถึง พฤศจิกายน พ.ศ. 2565 วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และสถิติวิลคอกซัน ผลการศึกษา: คะแนนความรู้ ทัศนคติ และการปฏิบัติของญาติผู้ดูแลหลังเข้าร่วมโปรแกรมเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (Z = -4.30, p < .001; Z = -4.03, p < .001; Z = -4.38, p < .001 ตามลำดับ) คะแนนคุณภาพการดูแลผู้ป่วยโดยรวมหลังเข้าร่วมโปรแกรมสูงกว่าก่อนเข้าร่วมโปรแกรมอย่างมีนัยสำคัญทางสถิติ (Z = -4.54, p < .001) สรุป: พยาบาลสามารถนำโปรแกรมการดูแลผู้ป่วยติดเตียงที่บ้าน ไปใช้ในการเตรียมความพร้อมและพัฒนาญาติผู้ดูแลให้มีความสามารถในดูแลผู้ป่วยติดเตียงที่บ้าน ส่งผลให้ผู้ป่วยติดเตียงได้รับการดูแลอย่างมีคุณภาพ และลดโอกาสการเกิดภาวะแทรกซ้อนที่ป้องกันได้
