The guidelines development on care and nursing for COVID-19 patients, Det Udom crown prince Hospital

Abstract

This action research was aimed to develop guideline of diagnosis and treatment for COVID-19 patient in Det Udom crown prince hospital. The system theory in aspect of input factor, process, and result, was applied for conceptual framework. Target group were consisted of medical doctor, nurse, and health personnel from the hospital and the district health office were participants for guideline development. The study was found, Guidelines for care of COVID-19 patient and how to diagnosis COVID-19 in case of suspected pneumonia/suspected COVID-19 pneumonia by revised definitions according to the Department of Disease Control. Define roles and duties ranging from screening system, coordination, and the epidemiology report to disease investigation. How to refer patient between various points, preparation the in-patient ward,  teaching and demonstration of wearing PPE and referral system at the district level Zone and Province. The dimension of the area were preparation the separation examination room of outpatient department, renovated the inpatient ward to be a room for receiving COVID-19 patients, creating a separate entrance to the ward of the staff and the patient's corridor, add a barrier gate between the corridors in the building, and dividing personnel and patients install an exhaust fan in the corridor. Medical equipment was prepared sufficiently supply.  And was found 51 patients under investigation (PUI) who were admitted from February 27, 2020 to June 24, 2020. 2 cases were confirmed COVID-19 patients and 6 cases with high risk exposure (HRC). No found on infection and transmission of COVID-19 among health personnel, patients and people in communities. An important for screen, surveillance, and prevention and control for disease transmission, Every health personnel must have knowledge of COVID-19 and understanding of the definitions and criteria of patients investigation in risky zone, guidelines for treatment and diagnosis, and the strengthening of the system on disease prevention and control. The success factor was depending on the director who response launch the budget and the policy of how to managing this situation in the hospital.
