Factors associated with injury severity from road traffic accidents among the elderly in Health Region 5, Thailand

Abstract

Road traffic accidents are a major cause of injury and mortality among older adults and have shown an increasing trend in Thailand. This analytical cross-sectional study aimed to identify factors associated with injury severity among elderly involved in road traffic accidents in Health Region 5. Data were collected from the Injury surveillance system of elderly who were injured in road traffic accidents and received treatment in emergency departments between January 1 and December 31, 2024. Data were analyzed using Multiple Logistic Regression, and the results were presented as Adjusted Odds Ratios (ORadj) with 95% Confidence Intervals (95% CI).

The results showed that, based on the analysis of 5,239 road traffic injury episodes among elderly with complete data, factors significantly associated with injury severity included alcohol consumption (ORadj = 1.46, 95% CI: 1.05–2.12), non-use of protective equipment (ORadj = 1.91, 95% CI: 1.54–2.38), and blunt injury mechanism (ORadj = 1.35, 95% CI: 1.10–1.65). Vehicle-related factors as well as road and environmental factors were not significantly associated with injury severity among elderly at the 0.05 significance level.

In conclusion, injury severity among elderly involved in road traffic accidents was associated with risk behaviors, particularly alcohol consumption, non-use of protective equipment, and blunt mechanisms of injury. Therefore, preventive measures should focus on reducing these risk behaviors, especially drink-driving control, strict enforcement of protective equipment laws, and the development of measures to reduce injury severity. Additionally, road safety education for elderly and the development of screening systems for elderly sustaining blunt injuries should be promoted to ensure timely assessment and appropriate management.
