การผ่าตัดต้อกระจกสุกแผลเล็กโดยไม่ใช้เครื่องสลาย ต้อกระจกด้วย Lerprat Technique

Abstract

ที่มาของปัญหา : ต้อกระจกเป็นสาเหตุอันดับหนึ่งที่ทำให้ตาบอด แต่สามารถรักษาให้หายได้ด้วยการผ่าตัด หาก ปล่อยไว้จนกลายเป็นต้อกระจกสุก อาจเสี่ยงต่อการเกิดโรคแทรกซ้อนและการผ่าตัดทำได้ยากยิ่งขึ้น

วัตถุประสงค์ : เพื่อนำเสนอการผ่าตัดต้อกระจกสุกแผลเล็กโดยไม่ใช้เครื่องสลายต้อกระจก (MSICS) ด้วย Lerprat technique

วิธีการศึกษา : เป็นเทคนิคการผ่าตัดที่บูรณาการมาจาก Ruit technique และ Modified Blumenthal technique โดยใช้เครื่องมือที่มีอยู่เดิมในการผ่าตัดต้อกระจกแบบแผลใหญ่ จะทำให้ไม่สิ้นเปลืองค่าใช้จ่ายในการ ซื้อหรือประดิษฐ์อุปกรณ์เพิ่มเติม

ผลการศึกษา : พบว่าผู้ป่วยที่เข้ารับการรักษาผ่าตัดต้อกระจกสุก จำนวน 10 ราย มีอัตราการมองเห็นที่ดีขึ้น โดยก่อนการผ่าตัดมีค่าการมองเห็นแย่กว่า 20/200 เมื่อวัดด้วย Snellen chart และหลังผ่าตัด มีค่าการ มองเห็นดีกว่า 20/40 ที่ 1 เดือนหลังผ่าตัด เมื่อวัดด้วย Snellen chart คิดเป็น 100% และมีการวัดเซลล์เยื่อบุผิวของกระจกตา (Endothelial cell count) จำนวน 5 ราย พบว่าก่อนการผ่าตัดมีค่าเฉลี่ย 2,478.4 cell/mm3 และหลังผ่าตัดที่ 1 เดือน มีค่าเฉลี่ย 2,357.4 cell/mm3 มีค่าลดลง ร้อยละ 4.88

สรุป : การผ่าตัดต้อกระจกสุกแผลเล็กโดยไม่ใช้เครื่องสลายต้อกระจก ด้วย Lerprat technique มีผลลัพธ์ เป็นที่น่าพอใจทั้งการมองเห็น และการวัดเซลล์เยื่อบุผิวของกระจกตา อย่างไรก็ตามในรายงานการ ศึกษาเบื้องต้นนี้ จำนวนผู้ป่วยที่ได้รับการผ่าตัดต้อกระจกด้วย Lerprat technique ยังมีจำนวนไม่มากนัก จำเป็นที่ต้องศึกษาในผู้ป่วยจำนวนมากขึ้น และติดตามผู้ป่วยเป็นระยะเวลานานขึ้นด้วย

Manual Sutureless Mature Cataract Surgery with Intraocular Lens using Lerprat Technique

Background : Cataract is the leading cause of blindness but it can be cured by performing an operation. However left untreated, the patient may stand the risk of having incurrent diseases in mature cataract, which can turn to a complicated surgery.

Objective : To study the Manual small incision cataract surgery (MSICS) using Lerprat technique in mature cataract.

Method : This novel technique is an adaptation from Ruit technique and Modified Blumenthal technique that allows ophthalmologist to use their existing Extracapsular cataract extraction surgery (ECCE) instruments to operate mature cataract without the need to purchase or invent new instrumens.

Results : Surgeries using Lerprat technique on mature cataract in 10 patients whose preoperative Visual acuity (VA) worse than 20/200 when measured by Snellen charts revealed that the patient VA was improved to better than 20/40 in one month postoperatively (10cases). When studies on Endothelial cell count (ECC) in 5 out of 8 patients also showed that, the average ECC decreased by 4.88 percent from 2478.4 cell/mm3 preoperation to 2357.4 cell/mm3 one month postoperatively.

Conclusion : This study concludes that the performance of MSICS using Lerprat technique in mature cataract was satisfactory when measured by VA and ECC. Nonetheless, as this novel technique had only been performed on a small number of patients, additional studies will be required.
