รายงานผู้ป่วยที่ได้รับการผ่าตัดต้อกระจกแผลเล็ก ด้วยเทคนิค Phaco-drainage ขนาดแผล 2.4 มม.

Abstract

วัตถุประสงค์ : เพื่อรายงานเทคนิคและผลการผ่าตัดในผู้ป่วยต้อกระจกสองราย ที่ได้รับการผ่าตัดต้อกระจกแบบแผลเล็ก (MSICS) โดยไม่ใช้ เครื่องสลายต้อคลื่นความถี่สูง ด้วยเทคนิค Phaco-drainage ขนาดแผล 2.4 มม.

วิธีการศึกษา : อธิบายเทคนิคการผ่าตัดและรายงานผู้ป่วยต้อกระจกที่ได้รับการผ่าตัด MSICS ด้วยเทคนิค Phaco-drainage สองรายที่ โรงพยาบาลเมตตาประชารักษ์ (วัดไร่ขิง) นำเสนอเปรียบเทียบระดับการมองเห็น จำนวนเซลล์เยื่อบุกระจกตา ค่าความเอียงของกระจกตาก่อนและหลังการผ่าตัด และภาวะแทรกซ้อนของการผ่าตัด

ผลการศึกษา : ผู้ป่วยทั้งสองรายไม่มีภาวะแทรกซ้อนของการผ่าตัด ระดับการมองเห็นดีขึ้นภายในสัปดาห์แรกของการผ่าตัด ที่หลังผ่าตัด 1 เดือน ผู้ป่วยมีระดับการมองเห็นเท่ากับ 20/16 และ 20/20 จำนวนเซลล์เยื่อบุกระจกตาลดลงร้อยละ 3.45 และร้อยละ 3.48 ที่ 1 เดือนหลังผ่าตัด ค่าความเอียงของกระจกตาหลังผ่าตัดมีการเปลี่ยนแปลงไปเพียงเล็กน้อยโดยก่อนผ่าตัด ค่าความเอียงของกระจกตา -0.50 x 3° และ -0.75 x 57° หลังผ่าตัด 1 สัปดาห์เท่ากับ -0.50 x 6° และ -0.50 x 69° หลังผ่าตัด 1 เดือนเท่ากับ -0.50° x 174° และ -0.75 x 49° ในผู้ป่วยรายที่ 1 และ 2 ตามลำดับ

สรุป : MSICS Phaco-drainage Technique เป็นวิธีการผ่าตัดที่ให้ผลดีในผู้ป่วยที่มีความเหมาะสม แต่ยังต้องการการศึกษาเพิ่มเติมต่อไป

Manual Small Incision Cataract Surgery (MSICS) through a 2.4 mm. Clear Cornea Incision with Phaco-drainage Technique: Two Case Reports.

Purpose: To report surgical technique and results in two cataract patients who underwent manual small incision cataract surgery through a 2.4 mm. clear cornea incision with Phaco-drainage tecnique

Material and Methods: This case report study comprised 2 patients with aged-related cataract that underwent manual small incision cataract surgery with Phaco-drainage technique in Mettapracharak Hospital. Pre and post operative evaluation was performed after 1st day, 1st week, 1st months. After operation postoperative endothelial cell count and loss, Uncorrected and Best-corrected visual acuity, pre and post-operative corneal astigmatism and postoperative complication were reported.

Results: Uncorrected and best-corrected visual acuity at 1st month was 20/16 and 20/20. Endothelial cell loss at 1st month was 3.45 percent and 3.48 percent respectively. Corneal astigmatism was minimally changed after 1st week and 1st month. Preoperative corneal astigmatism was -0.50 x 3° and -0.75 x 57°, postoperative 1 week was -0.50 x 6° and -0.50 x 69°, Postoperative 1st month was -0.50° x 174° and -0.75 x 49° respectively. No postoperative complication was found.

Conclusion : MSICS Phaco-drainage technique achieved good visual outcomes, and offered potential benefits in mild to moderate cataract. Further additional research should be performed.
