Melioidosis in Prapokklao Hospital,Thailand.

Abstract

Objective : Melioidosis is regarded as endemic infection in Southeast Asia and Northern Australia.In Thailand,melioidosis have found in every region.This retrospective study reviews clinical manifestration,underlying disease and result of treatment of melioidosis cases in Prapokklao hospital,Chanthaburi,Thailand.

Patients & Method : The patients who had positive culture of Burkhoderia pseudomallei from the laboratory records and were admitted at Prapokklao Hospital during January 1,2003 to December 31,2006 were enrolled in the study.

Results : Fifty-eight patients with melioidosis infection were reviewed.Two patients were infant and one patient had recurrent infection.Clinical manifestion included disseminated septicemia,non-disseminated septicemia and localized infection 3,42 and 13 patients,respectively.Respiratory tract infection was the most common site of infection (36 patients,31 patients had pneumonia, 5 patients had pleural effusion) follow by joint infection (11 patients).Most patient had underlying disease (diabetes 27 patients (46.6%),chronic renal failure 9 patients (15.5%) and chronic liver disease 9 patients (15.5%).Fufty patients (86.2%) were received appropriated antibiotic.Mortality rate in patients who received appropriated antibiotic and inappropriated antibiotic was 22% and 62.5% respectively.

Conclusion : We found melioidosis infection in patients with diabetes,chronic renal failure and chronic liver disease.Patients who received appropriated antibiotic had less mortality rate than received inappropriated antibiotic significantly (P value=0.03).
