 ปัญหาโรคไม่ติดต่อเรื้อรังยังคงเป็นปัญหาสุขภาพที่สำคัญในระบบสุขภาพของประเทศไทย คลินิกโรคไม่ติดต่อเรื้อรังในโรงพยาบาลส่งเสริมสุขภาพตำบลเป็นอีกหนึ่งหน่วยบริการที่มีสำคัญมากขึ้นตามจำนวนผู้ป่วยที่เพิ่มมากขึ้นในชุมชน จึงเป็นความท้าทายของพยาบาลเวชปฏิบัติที่ต้องพัฒนาคลินิกโรคไม่ติดต่อเรื้อรังในโรงพยาบาลส่งเสริมสุขภาพตำบลให้มีคุณภาพสอดคล้องกับความต้องการด้านสุขภาพของผู้รับบริการ ชุมชน และนโยบายของกระทรวงสาธารณสุข กรอบแนวคิด 7s ของแมคเคนซีย์ (The McKinsey 7s framework) เป็นแนวคิดในการพัฒนารูปแบบขององค์กรโดยเน้นการสร้างความสัมพันธ์ของปัจจัยต่างๆ ในองค์กรทั้ง 7 ด้านโดยยึดหลักการมีส่วนร่วมในทุกระดับและการสร้างค่านิยมร่วมกันเป็นสำคัญ จึงอาจเป็นอีกหนึ่งเครื่องมือที่จะช่วยให้พยาบาลเวชปฏิบัติสามารถประเมินหน่วยงานของตนเองในแง่มุมต่างๆ ร่วมกับทีมสหวิชาชีพและผู้มีส่วนได้ส่วนเสียในชุมชน เพื่อนำไปสู่การพัฒนาคลินิกโรคไม่ติดต่อเรื้อรังในโรงพยาบาลส่งเสริมสุขภาพตำบลให้ที่มีคุณภาพต่อไป

The Development of a Quality Non-Communicable Disease Clinic in Sub-district Health Promotion Hospitals Based on the McKinsey 7s framework: A Challenging Role of Nurse Practitioners

Abstract

Non-communicable diseases (NCDs) still remain a significant health problem in Thailand. An NCD clinic in sub-district health promotion hospital has become more important as the number of NCD patients in community has grown. This results in a challenge for nurse practitioners who work in any sub-district health promotion hospital since they take a major role in improving the quality of an NCD clinic to serve client’s needs and fulfill the policy of the Ministry of Public Health. The McKinsey 7s framework is a concept of organizational development where 7 organizational factors are taken into account with a particular concern of organizational participation and core values. Therefore, it might be one of the means to conceive health setting evaluation, which is necessary for the quality improvement of NCD clinics in Sub-District Health Promotion Hospitals.
