บทคัดย่อ
ที่มาของปัญหา: องค์การอนามัยโลก พ.ศ. 2543 พบทั่วโลกมีผู้ป่วยเบาหวาน 171 ล้านคน มีแนวโน้มเพิ่มขึ้นเป็น 366 ล้านคน ใน พ.ศ. 2573 สมาพันธ์เบาหวานนานาชาติ พ.ศ. 2558 พบผู้ป่วยเบาหวานทั่วโลก 415 ล้านคน จะเพิ่มขึ้นเป็น 642 ล้านคนใน พ.ศ. 2583 ประเทศไทยพบว่ามีแนวโน้มจำนวนผู้ป่วยเบาหวานเพิ่มมากขึ้น จากร้อยละ 2.3 ใน พ.ศ. 2534 เป็นร้อยละ 6.9 ใน พ.ศ. 2552 เมื่อผ่านไป 15 ปี พบว่า ผู้ป่วยเบาหวานมีภาวะตาบอด ร้อยละ 2 สายตาเลือนราง ร้อยละ 10
วัตถุประสงค์: หาความครอบคลุมการคัดกรองเบาหวานขึ้นจอประสาทตา และความชุกของเบาหวานขึ้นจอประสาทตา เขตบริการสุขภาพเขต 6 ปีงบประมาณ 2559
วัสดุและวิธีการศึกษา: พรรณนาแบบย้อนหลัง
ผลการศึกษา: ผู้ป่วยได้รับการวินิจฉัยโรคเบาหวานทั้งหมด 191,776 คน เข้ารับการตรวจจอประสาทตา 85,786 คน คิดเป็นความครอบคลุม ร้อยละ 44.73 จังหวัดที่มีความครอบคลุมการตรวจเบาหวานขึ้นจอประสาทตามากสุด คือ ระยอง (ร้อยละ 73.54) ปราจีนบุรี (ร้อยละ 71.10) และจันทบุรี (ร้อยละ 68.96) พบความผิดปกติจอประสาทตาจำนวน 12,430 คน คิดเป็นความชุกของเบาหวานขึ้นจอประสาทตา ร้อยละ 14.48 ภาวะเบาหวานขึ้นจอประสาทตาชนิดไม่รุนแรง (NPDR) 10,437 คน (ร้อยละ 12.16) ภาวะเบาหวานขึ้นจอประสาทตาชนิดรุนแรง (PDR) 1,719 คน (ร้อยละ 2.00) จังหวัดที่พบเบาหวานขึ้นจอประสาทตามากที่สุด คือ ชลบุรี (ร้อยละ 25.80) สมุทรปราการ (ร้อยละ 20.87) และตราด (ร้อยละ 19.07)
สรุป: ภาวะเบาหวานขึ้นจอประสาทตา พบได้บ่อยในผู้ป่วยเบาหวาน ทำให้สูญเสียการมองเห็น ดังนั้นการตรวจคัดกรองจอประสาทตาในผู้ป่วยเบาหวาน จะทำให้มีความชุกของการเกิดโรคได้น้อยลง เพราะจะสามารถตรวจพบและรักษาโรคได้อย่างรวดเร็ว และยังลดภาวะแทรกซ้อนที่จะเกิดขึ้นจากภาวะเบาหวานขึ้นจอประสาทตาได้ด้วย
