บทคัดย่อ
อัตราการผ่าตัดคลอดบุตรทางหน้าท้องสูงขึ้นมากในทุกสถานพยาบาล แม้ว่าองค์การอนามัยโลกจะกำหนดตัวเลขที่เหมาะสมว่าควรเป็นร้อยละ 10-15 ความพยายามในการลดอัตราการผ่าตัดคลอด สามารถทำได้โดยการลดการผ่าตัดคลอดบุตรปฐมภูมิ และทุติยภูมิ รวมทั้งการให้ความรู้ที่แท้จริงแก่ประชาชน การสร้างจิตสำนึกแก่สูตินรีแพทย์ และความร่วมมือขององค์กรเอกชนในทุกภาคส่วน ซึ่งอาจจะไม่สำเร็จได้ในเร็ววัน อย่างไรก็ดีสิ่งที่สำคัญกว่าตัวเลขของ “อัตราการผ่าคลอด” คือเป้าหมายนั้นต้องเป็นไปเพื่อผลลัพธ์ทางสุขภาพที่ดีที่สุดของมารดาและทารก โดยคำนึงถึงทรัพยากรทางสาธารณสุขที่มีร่วมกับความพึงพอใจของมารดา ดังนั้น ตัวเลขอัตราการผ่าคลอดจึงแปรเปลี่ยนไปในแต่ละบุคคลตามกาลเวลา ประชากร และสภาพสังคม
