บทคัดย่อ


ที่มาของปัญหา: ภาวะหลอดเลือดดำในสมองอุดตันเป็นภาวะที่พบเป็นสาเหตุของโรคหลอดเลือดสมองน้อยกว่าร้อยละ 1 ของโรคหลอดเลือดสมองทั้งหมด โดยทั่วไปมักจะวินิจฉัยภาวะนี้ได้ล่าช้า เนื่องจากผู้ป่วยมาด้วยอาการหลากหลาย และการวินิจฉัยที่ล่าช้านี้อาจนำมาสู่การเสียชีวิตของผู้ป่วยได้


วัตถุประสงค์: เพื่อหาปัจจัยที่ทำให้การวินิจฉัยภาวะหลอดเลือดดำในสมองอุดตันล่าช้า อัตราพิการและอัตราตายของผู้ป่วยภาวะหลอดเลือดดำในสมองอุดตัน


วิธีการศึกษา: เป็นการศึกษาแบบย้อนหลัง โดยเก็บข้อมูลผู้ป่วยภาวะหลอดเลือดดำในสมองอุดตัน ที่เข้ารับการรักษาในโรงพยาบาลพระปกเกล้า ระหว่างเดือนมกราคม 2556 ถึงเดือนธันวาคม 2560


ผลการศึกษา: พบผู้ป่วยภาวะหลอดเลือดดำในสมองอุดตัน 39 ราย เป็นเพศหญิง 24 ราย (ร้อยละ 61.5) เพศชาย 15 ราย (ร้อยละ 38.5) ค่ามัธยฐานของอายุคือ 37 ปี ระยะเวลาตั้งแต่ผู้ป่วยมาโรงพยาบาลจนได้รับการวินิจฉัยน้อยกว่า 24 ชั่วโมง 12 ราย, 24-48 ชั่วโมง 13 ราย และมากกว่า 48 ชั่วโมง 14 ราย มีผู้ป่วยเพียงร้อยละ 23 ได้รับการวินิจฉัยถูกต้องว่ามีภาวะหลอดเลือดดำในสมองอุดตันตั้งแต่แรกรับ ผู้ป่วยที่มี Glasgow coma score น้อยกว่า 12 คือปัจจัยเดียวที่สัมพันธ์กับการวินิจฉัยล่าช้า ปัจจัยเสี่ยงที่พบบ่อยที่สุดในเพศหญิงคือรับประทานยาคุมกำเนิด ผู้ป่วยเพศชายส่วนใหญ่หาปัจจัยเสี่ยงไม่พบ (8/15 ราย) พบอัตราพิการร้อยละ 20.5 อัตราเสียชีวิตร้อยละ 10.3 ซึ่งไม่มีความแตกต่างกันระหว่างผู้ป่วยทั้ง 3 กลุ่ม


สรุป: ผู้ป่วยหญิงที่มาด้วยอาการปวดศีรษะ หรืออาการผิดปกติอื่นของระบบประสาท โดยเฉพาะผู้ป่วยที่รับประทานยาคุมกำเนิดและมีภาวะเลือดออกในสมองที่ยังหาสาเหตุไม่พบ ควรคิดถึงภาวะหลอดเลือดดำในสมองอุดตัน
