การสอบสวนผู้ป่วยติดเชื้อ Vibrio cholerae non O1/ non O139 ในกระแสเลือด จังหวัดสุรินทร์ 1 ราย ปี 2560

บทคัดย่อ
ผู้ป่วยหญิง 40 ปี เข้ารักษาที่โรงพยาบาลสุรินทร์ และเสียชีวิตจากการติดเชื้อ Vibrio cholerae non O1/non O139 ในกระแสเลือด หลังจากได้รับรายงานในวันที่ 14 เดือนเมษายน พ.ศ. 2560 ทีม SRRT ได้ออกสอบสวนการระบาดเฉพาะรายกรณีในวันที่ 11 - 22 เดือนเมษายน พ.ศ. 2560 ศึกษาโดยใช้ระบาดวิทยาเชิงพรรณ และได้ศึกษาในผู้เกี่ยวข้องทุกราย วัตถุประสงค์เพื่อยืนยันกาวินิจฉัยโรค การระบาดโรค และกำหนดแนวทางควบคุมการระบาดของโรค โดยใช้เครื่องมือแบบสอบสวนโรค ทะเบียนประวัติ ใบชันสูตร ผู้สัมผัสร่วมบ้าน 3 ราย ได้รับการตรวจ Rectal swab culture และเก็บตัวอย่างวัตถุดิบในการประกอบอาหาร 1 ตัวอย่าง น้ำดื่ม ภาชนะบรรจุน้ำดื่มและภาชนะบรรจุอาหารทุกตัวอย่างให้ผล Negative for Vibrio spp. ผู้ป่วยไม่มีประวัติออกนอกพื้นที่ ไม่มีประวัติสัมผัสผู้สงสัยว่ามีการติดเชื้อมาก่อนสรุปสาเหตุของการเสียชีวิตของผู้ป่วยในครั้งนี้เกิดจากโรคประจำตัว คือ ตับแข็งและการทำงานของหลายอวัยวะล้มเหลว ที่เป็นปัจจัยต่อการเกิดโรค ทีม SRRT ได้เฝ้าระวังต่อ 12 วันไม่พบรายใหม่เพิ่มและในอำเภอเมืองสุรินทร์ไม่เคยพบว่ามีผู้ป่วย Vibrio cholerae ใน 5 ปีที่ผ่านมา
