ผลของการผ่าตัดต้อกระจกโดยวิธีสลายต้อกระจกต่อความดันตาในผู้ป่วยต้อหินมุมปิดปฐมภูมิ ของโรงพยาบาลพนัสนิคม จังหวัดชลบุรี

บทคัดย่อ

ที่มาของปัญหา: โรคต้อหินมุมปิดเป็นสาเหตุสำคัญที่ทำให้ผู้ป่วยต้อหินตาบอดในประเทศไทยและทั่วโลก ส่งผลกระทบต่อภาพรวมทางด้านสุขภาพและระบบสาธารณสุขของประเทศ แนวทางการรักษาในแต่ละระยะของโรคยังมีความแตกต่างกัน การค้นหาปัจจัยที่ส่งผลต่อการรักษาจึงเป็นสิ่งสำคัญเพื่อวางแผนการดูแลผู้ป่วยต่อไป


วัตถุประสงค์: เพื่อศึกษาผลของการผ่าตัดต้อกระจกโดยวิธีสลายต้อกระจกต่อความดันตาและค้นหาปัจจัยที่ส่งผลต่อความดันตาหลังผ่าตัดต้อกระจกในผู้ป่วยต้อหินมุมปิดปฐมภูมิ


วิธีการศึกษา: เป็นการศึกษาเชิงสังเกตแบบการศึกษาย้อนหลัง ระยะเวลาในเก็บข้อมูล 5 ปี ระหว่างวันที่ 1 ตุลาคม พ.ศ.2560 ถึงวันที่ 30 กันยายน พ.ศ.2565 ในผู้ป่วยต้อหินมุมปิดปฐมภูมิที่ได้รับการผ่าตัดต้อกระจกโดยวิธีสลายต้อกระจก จำนวน 121 คน แบบบันทึกข้อมูลก่อนผ่าตัดและหลังผ่าตัด ได้แก่ ระดับการมองเห็น ความดันตา จำนวนยาต้อหิน ค่า parameter ต่างๆในลูกตา และวิเคราะห์ข้อมูล โดยใช้สถิติพรรณนา Paired sample t-test, Repeated measures ANOVA และ Multiple linear regression กำหนดค่านัยสำคัญทางสถิติที่ 0.05


ผลการศึกษา: ผู้ป่วยที่ได้รับการผ่าตัดจำนวน 121 คน ส่วนใหญ่เป็นเพศหญิงคิดเป็นร้อยละ 79.3 อายุเฉลี่ยทั้งหมด 67.9±8.4 ปี ความดันตาเฉลี่ยลดลงหลังผ่าตัดอย่างมีนัยสำคัญ จากความดันตาเริ่มต้น 17.9±5.1 มิลลิเมตรปรอท ลดลงเป็น 12.1±2.5 มิลลิเมตรปรอท (p<0.001) กลุ่มที่มีความดันตาสูงกว่าก่อนผ่าตัดมีการลดลงของความดันตาหลังผ่าตัดมากกว่าอย่างมีนัยสำคัญ (p<0.001) ระดับการมองเห็นหลังผ่าตัดดีขึ้นอย่างมีนัยสำคัญ (logMAR ลดลง 0.8, p<0.001) จำนวนยาต้อหินที่ใช้หลังผ่าตัดลดลงอย่างมีนัยสำคัญ (p<0.001) โดยปัจจัยทำนายที่มีผลต่อการลดลงความดันตาหลังผ่าตัด ได้แก่ ค่าความดันตาก่อนผ่าตัด (β=0.86, p<0.001)


สรุป: การผ่าตัดต้อกระจกโดยวิธีสลายต้อกระจกในผู้ป่วยต้อหินมุมปิดปฐมภูมิส่งผลให้ความดันตาลดลงหลังผ่าตัด มีระดับการมองเห็นที่ดีขึ้น มีจำนวนยาต้อหินที่ใช้หลังผ่าตัดลดลง และความดันตาก่อนผ่าตัดเป็นปัจจัยที่ส่งผลต่อการลดลงของความดันตาหลังผ่าตัด อย่างไรก็ตามควรต้องมีการศึกษาเกี่ยวกับ parameter ต่างๆ ในลูกตาเพิ่มเติมเพื่อใช้ในการทำนายค่าความดันตาหลังผ่าตัด เป็นประโยชน์ในการวางแผนการรักษาผู้ป่วยให้ดีมากขึ้นต่อไป
