บทคัดย่อ

ที่มาของปัญหา: โรคเบาหวานเป็นปัญหาสุขภาพสำคัญของคนไทย ผู้ป่วยจำนวนมากไม่สามารถควบคุมระดับน้ำตาลได้ ส่งผลให้เกิดภาวะแทรกซ้อนเพิ่มขึ้น ในปี พ.ศ. 2565 โรงพยาบาลสมเด็จพระบรมราชเทวี ณ ศรีราชา ได้จัดตั้งคลินิกโรคไม่ติดต่อเรื้อรัง (Modified Non-communicable Diseases; Modified NCDs) เพื่อให้การดูแลผู้ป่วยเบาหวานแบบครบวงจร ซึ่งแตกต่างจากคลินิกเวชปฏิบัติทั่วไป (GP) ในด้านโครงสร้างบุคลากรและรูปแบบบริการ


วัตถุประสงค์: เพื่อเปรียบเทียบระดับน้ำตาลสะสม (HbA1C) ของผู้ป่วยเบาหวานชนิดที่ 2 ระหว่างคลินิก Modified NCDs และคลินิก GP


วิธีการศึกษา: การศึกษาย้อนหลังแบบติดตามไปข้างหน้า (Retrospective cohort study) ในผู้ป่วยเบาหวานชนิดที่ 2 จำนวน 152 ราย แบ่งเป็นกลุ่ม Modified NCDs 85 ราย และกลุ่ม GP 67 ราย ติดตามทุก 3 เดือน เป็นระยะเวลา 1 ปี เก็บข้อมูลประชากร โรค ข้อมูลทางคลินิก และการคัดกรองภาวะแทรกซ้อนจากเวชระเบียนย้อนหลัง


ผลการศึกษา: ลักษณะพื้นฐานของผู้ป่วย เช่น อายุ โรคร่วม ระดับ HbA1C และน้ำตาลหลังอดอาหาร ไม่แตกต่างกันอย่างมีนัยสำคัญ กลุ่ม Modified NCDs มีสัดส่วนการควบคุม HbA1C ได้ตามเป้าหมายสูงกว่ากลุ่ม GP อย่างมีนัยสำคัญ (ร้อยละ 49.4 vs. ร้อยละ 31.3, p=0.025) และมีอัตราการคัดกรองภาวะแทรกซ้อนสูงกว่า ได้แก่ ภาวะจอตาผิดปกติ (ร้อยละ 97.7 vs. ร้อยละ 79.1, p<0.001) โรคไตจากเบาหวาน (ร้อยละ 91.8 vs. ร้อยละ 74.6, p=0.013) และเส้นประสาทส่วนปลายเสื่อม (ร้อยละ 81.1 vs. ร้อยละ 0, p<0.001)


สรุป: คลินิก Modified NCDs ให้ผลลัพธ์ดีกว่าในการควบคุมระดับน้ำตาลและการคัดกรองภาวะแทรกซ้อน สะท้อนประสิทธิภาพของการดูแลแบบบูรณาการและต่อเนื่อง ซึ่งอาจเป็นแนวทางสำคัญในการพัฒนาระบบการดูแลผู้ป่วยโรคเรื้อรังในอนาคต
