บทคัดย่อ

ที่มาของปัญหา: ผู้ป่วยฟอกเลือดมีความเสี่ยงต่อปัญหาสุขภาพจิตสูงกว่าประชากรทั่วไป จำเป็นต้องศึกษาปัจจัยที่มีผลต่อสุขภาวะทางจิตใจ เพื่อพัฒนารูปแบบการดูแลแบบองค์รวม


วัตถุประสงค์: ศึกษาปัจจัยที่มีผลต่อสุขภาวะทางจิตใจและประสบการณ์ทางจิตใจของผู้ป่วยฟอกเลือดด้วยเครื่องไตเทียม


วิธีการศึกษา: ใช้วิจัยแบบผสมผสานชนิดดำเนินการพร้อมกัน กลุ่มตัวอย่างเชิงปริมาณจำนวน 100 ราย และเชิงคุณภาพจำนวน 15 ราย เครื่องมือประกอบด้วย แบบบันทึกข้อมูลส่วนบุคคลและความเจ็บป่วย แบบประเมินสุขภาวะทางจิตใจทั่วไป ความยืดหยุ่นทางจิตใจ ความเชื่อในอำนาจการควบคุมด้านสุขภาพ และแบบสัมภาษณ์เชิงลึกแบบกึ่งโครงสร้าง ค่าดัชนีความตรงเชิงเนื้อหา 0.81-1.00 และค่าความเชื่อมั่น 0.76-0.97 วิเคราะห์ด้วยสถิติเชิงพรรณนา สหสัมพันธ์แบบเพียร์สัน การถดถอยพหุคูณ และการวิเคราะห์เนื้อหา


ผลการศึกษา: ความยืดหยุ่นทางจิตใจ ระดับฮีโมโกลบิน และความเชื่อในอำนาจการควบคุมด้านสุขภาพ สามารถร่วมกันอธิบายความแปรปรวนของสุขภาวะทางจิตใจได้ร้อยละ 16.9 โดยความยืดหยุ่นทางจิตใจเป็นตัวแปรที่มีอำนาจในการทำนายสูงสุด (b=0.289, 95% CI [0.019, 0.559], p<0.05). ส่วนข้อมูลเชิงคุณภาพสังเคราะห์ได้เป็น 3 ธีมหลัก ได้แก่ ความทุกข์ทางอารมณ์จากข้อจำกัดและการสูญเสียบทบาท กระบวนการเปลี่ยนผ่านทางจิตใจจากการต่อต้านสู่การยอมรับ และบทบาทของความสัมพันธ์และระบบสนับสนุนทางสังคม


สรุป: ประสบการณ์ทางจิตใจของผู้ป่วยที่รับการฟอกเลือดด้วยเครื่องไตเทียมมีลักษณะซับซ้อนและเปลี่ยนแปลงไปตามระยะเวลาของการรักษา ดังนั้น จึงควรพัฒนาระบบการประเมินสุขภาวะทางจิตใจอย่างต่อเนื่องและเป็นระบบ ควบคู่ไปกับการจัดการภาวะโลหิตจางอย่างมีประสิทธิภาพ รวมถึงส่งเสริมความยืดหยุ่นทางจิตใจให้แก่ผู้ป่วยในทุกระยะของการดูแล
