การสร้างคะแนนทางคลินิกเพื่อทำนายการรักษาด้วยยาต้านไทรอยด์ไม่ได้ผลในผู้ป่วยโรคไทรอยด์เป็นพิษ(Graves’ disease)

บทคัดย่อ
ที่มาของปัญหา: การรักษา Graves’ disease มี 3 วิธี ได้แก่ Antithyroid drug, Radioactive iodine และThyroidectomy ทางเลือกในการรักษา ขึ้นอยู่กับลักษณะทางคลินิก แม้การให้ยาต้านไทรอยด์ในระยะยาวเป็นอีกหนึ่งทางเลือก แต่พบว่ามีผู้ป่วยส่วนหนึ่งไม่หายขาด
วัตถุประสงค์: เพื่อนำลักษณะทางคลินิกมาสร้างเป็นคะแนนทำนายผลการรักษาด้วยยาต้านไทรอยด์ไม่ได้ผลในผู้ป่วย Graves’disease
วิธีการศึกษา: การศึกษา Prognostic prediction research รูปแบบ Retrospective observational cohort design ในผู้ป่วย Graves’ disease ที่โรงพยาบาลสุรินทร์ พ.ศ. 2561-2568 จำนวน 630 ราย ศึกษาข้อมูลจากเวชระเบียนผู้ป่วยนอก บันทึกข้อมูลลักษณะทางคลินิก และสร้้างชุุดตััวแปรทำนายการรักษาด้วยยาต้านไทรอยด์ไม่ได้ผลด้วย Multivariable logistic regression โดยกำหนดคะแนนทำนายในแต่ละปัจจัยจากค่า Coefficient ของตััวแปร นำมารวมกันเป็นคะแนนทำนาย
ผลการศึกษา: ผู้ป่วย Graves’ disease 630 ราย จำแนกเป็นกลุ่มที่รักษาด้วยยาต้านไทรอยด์ไม่ได้ผล 306 ราย และได้ผล 324 ราย ปัจจัยการทำนายที่ Significant 5 อย่าง ได้แก่ ประวัติ Relapsed ขนาดต่อมไทรอยด์≥45 กรัม มีอาการทางตา FT4>3.5 ng/dl และ MMI dose ที่ 6 เดือนมากกว่าหรือเท่ากับ 10 มิลลิกรัม นำมาสร้างคะแนนทำนาย ได้คะแนนตั้งแต่ 0-7 แบ่งผู้ป่วยเป็น 2 กลุ่ม คือ ความเสี่ยงต่ำ (0-4 คะแนน) (PPV=38.9%) และความเสี่ยงสูง (4.5-7 คะแนน) (PPV=81.3%) ต่อการรักษาด้วยยาต้านไทรอยด์ไม่ได้ผล คะแนนดังกล่าวใช้ทำนายความเสี่ยงได้ดี (AuROC 0.78, 95% CI 0.74-0.82)
สรุป: ผู้ป่วย Graves’ disease ที่มีค่าคะแนนการทำนายการรักษาด้วยยาต้านไทรอยด์ไม่ได้ผลตั้งแต่ 4.5-7คะแนน มีความเสี่ยงสูงในการรักษาด้วยยาไม่ได้ผล แพทย์ควรแนะนำให้ผู้ป่วยกลุ่มนี้รักษาโดยการกลืนแร่หรือผ่าตัด
