บทคัดย่อ
ที่มาของปัญหา: การถ่ายโอนภารกิจตามรัฐธรรมนูญการกระจายอำนาจ พ.ศ.2543 พบปัญหา อุปสรรค และข้อจำกัดในการดำเนินงานหลายประการ
วัตถุประสงค์: เพื่อศึกษาสถานการณ์ และพัฒนาแนวทางในการบูรณาการระบบการบริการสุขภาพปฐมภูมิระหว่างสำนักงานสาธารณสุขจังหวัดและองค์การบริหารส่วนจังหวัดจันทบุรี
วิธีการศึกษา: วิจัยแบบผสมผสานวิธี (Mix Methods Research) รูปแบบ Explanatory Sequential Design นี้ เก็บข้อมูลโดยใช้แบบสอบถาม และข้อมูลการสนทนากลุ่มผู้ปฎิบัติงานเมื่อผ่านการรับรองจริยธรรมการวิจัยในมนุษย์ กลุ่มตัวอย่างสุ่มอย่างง่าย จำนวน 342 คน และข้อมูลเชิงคุณภาพ ด้วยเทคนิค Focus Group Discussion จำนวน 29 คน วิเคราะห์ข้อมูลโดยสถิติเชิงพรรณา ได้แก่ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน สถิติ ANOVA และวิคราะห์เชิงเนื้อหา ระยะเวลาศึกษาระหว่าง ตุลาคม 2567 - เมษายน 2568
ผลการศึกษา: ผลการวิเคราะห์เชิงปริมาณสนับสนุนข้อค้นพบเชิงคุณภาพอย่างมีนัยสำคัญ โดยพบความแตกต่าง ด้านการรับรู้ระหว่างหน่วยงานในมิติบทบาทหน้าที่ กำลังคน และตัวชี้วัด (p<0.001) แนวทางการบูรณาการทั้งหมดมุ่งสู่คุณภาพบริการและการเข้าถึงที่เท่าเทียมของประชาชน ในลักษณะโครงสร้างการกำกับร่วม (Dual Governance Structure) โดยสำนักงานสาธารณสุขจังหวัดทำหน้าที่กำกับเชิงวิชาการและมาตรฐาน ส่วนองค์การบริหารส่วนจังหวัดรับผิดชอบการบริหารจัดการเชิงปฏิบัติ ภายใต้กลไกธรรมาภิบาลร่วมอย่างชัดเจน เชื่อมโยงองค์ประกอบทั้ง 6 ด้านของระบบสุขภาพตามกรอบ Six Building Blocks ควบคู่กับการพัฒนาตามหลักการ Primary Health Care
สรุป: กรอบการบูรณาการระบบบริการสุขภาพปฐมภูมิภายหลังการถ่ายโอน จัดวางโครงสร้างให้เห็นความสัมพันธ์ของบทบาทเชิงกำกับ และบทบาทเชิงบริหารอย่างชัดเจน การนำผลไปใช้ควรพิจารณาข้อจำกัดดังกล่าวควบคู่กัน เพื่อให้เกิดความเหมาะสมกับบริบทและสถานการณ์ที่แตกต่างกันในแต่ละพื้นที่
