ความสัมพันธ์ระหว่างภาวะหมดไฟในการทำงานกับกลุ่มอาการเมตาบอลิกในบุคลากรโรงพยาบาลเอกชนแห่งหนึ่ง ในจังหวัดสงขลา

บทคัดย่อ

ที่มาของปัญหา: บุคลากรโรงพยาบาลเอกชนต้องปฏิบัติงานภายใต้แรงกดดันในระดับสูง ส่งผลให้เกิดภาวะหมดไฟในการทำงาน (burnout) ซึ่งอาจสัมพันธ์กับความผิดปกติทางเมตาบอลิซึมผ่านกลไกของความเครียดเรื้อรัง อย่างไรก็ตาม หลักฐานเชิงประจักษ์เกี่ยวกับความสัมพันธ์ระหว่างภาวะหมดไฟในการทำงานกับกลุ่มอาการเมตาบอลิกในบุคลากรโรงพยาบาลเอกชนในประเทศไทยยังมีจำกัด


วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์ระหว่างภาวะหมดไฟในการทำงานกับกลุ่มอาการเมตาบอลิก รวมถึงองค์ประกอบย่อยของกลุ่มอาการเมตาบอลิก ในบุคลากรโรงพยาบาลเอกชนแห่งหนึ่ง


วิธีการศึกษา: การวิจัยเชิงวิเคราะห์แบบภาคตัดขวางย้อนหลัง โดยใช้ข้อมูลทุติยภูมิจากผลการตรวจสุขภาพประจำปีและข้อมูลการประเมินภาวะหมดไฟในการทำงานของบุคลากรโรงพยาบาลจำนวน 477 คน ภาวะหมดไฟในการทำงานประเมินโดยใช้แบบประเมิน Maslach Burnout Inventory (MBI) และกลุ่มอาการเมตาบอลิกวินิจฉัยนิยามตามเกณฑ์ Harmonized Definition ปี ค.ศ. 2009 วิเคราะห์ความสัมพันธ์ด้วยการถดถอยโลจิสติกพหุตัวแปร โดยปรับปัจจัยกวน


ผลการศึกษา: ความชุกของภาวะหมดไฟในการทำงานและกลุ่มอาการเมตาบอลิกเท่ากับร้อยละ 15.5 และ 24.7 ตามลำดับ ไม่พบความสัมพันธ์ระหว่างภาวะหมดไฟในการทำงานกับกลุ่มอาการเมตาบอลิก (adjusted odds ratio=1.53; 95%CI: 0.86–2.70) และไม่พบความสัมพันธ์กับองค์ประกอบย่อยทั้งห้าประการของกลุ่มอาการเมตาบอลิกเช่นเดียวกัน ภายหลังการปรับปัจจัยกวน


สรุป: ภาวะหมดไฟในการทำงานไม่พบความสัมพันธ์กับกลุ่มอาการเมตาบอลิกในประชากรที่ศึกษา ความสัมพันธ์ดังกล่าวอาจเป็นความสัมพันธ์ทางอ้อม หรือจำเป็นต้องอาศัยการสะสมของความเครียดเรื้อรังเป็นระยะเวลานาน การศึกษาเชิงติดตามในอนาคตจึงมีความจำเป็นเพื่ออธิบายกลไกที่เกี่ยวข้องให้ชัดเจนยิ่งขึ้น
