ผลกระทบของการขาดนัดติดตามผู้ป่วยนอกต่อการเกิดเหตุการณ์ไม่พึงประสงค์ทางหลอดเลือดหัวใจใน 1 ปี ในผู้ป่วย STEMI ความเสี่ยงต่ำภายใต้ระบบ Shared-Care: การศึกษาแบบ Retrospective Nested Matched Case-Control

บทคัดย่อ
ที่มาของปัญหา: ผู้ป่วย STEMI ที่มี left ventricular ejection fraction (LVEF หรือค่าการบีบตัวของหัวใจห้องล่างซ้าย) ≥50% ภายหลัง primary PCI ถูกจัดในกลุ่มความเสี่ยงต่ำ อย่างไรก็ตาม ยังพบ MACE ในอัตราร้อยละ 10–15 ภายใน 1 ปี ในระบบ Shared-Care ของไทย การขาดนัดระหว่างการเปลี่ยนถ่ายการดูแลสู่โรงพยาบาลทุติยภูมิอาจเป็นสัญญาณเตือนระดับระบบที่ถูกมองข้าม
วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์ระหว่างการขาดนัดนานกว่า 60 วัน กับ MACE ภายใน 1 ปีหลัง STEMI ในผู้ป่วย LVEF≥50% ภายใต้ระบบ Shared-Care
วิธีการศึกษา: Retrospective nested matched case-control study ณ โรงพยาบาลพัทยาปัทมคุณ จังหวัดชลบุรี มกราคม พ.ศ. 2563 ถึง ธันวาคม พ.ศ. 2568 กลุ่มศึกษาคือผู้ป่วยที่เกิด MACE ภายใน 365 วันหลังจำหน่าย กลุ่มควบคุมคัดเลือกจาก ผู้ป่วยที่เข้าเกณฑ์การศึกษา 203 ราย จับคู่ด้วยเพศและอายุ (±5 ปี) แบบอัตราส่วนแปรผัน (1:7–1:10) ได้กลุ่มวิเคราะห์ 100 ราย (cases 10 ราย, controls 90 ราย) วิเคราะห์ด้วย exact conditional logistic regression สร้าง 2 แบบจำลอง: Model 1 (ปรับด้วยโรคเบาหวานและ IV furosemide ภายใน 24 ชั่วโมง เป็นตัวแทน Killip class) และ Model 2 (ปรับด้วยโรคเบาหวาน)
ผลการศึกษา: กลุ่มตัวอย่างที่วิเคราะห์ 100 ราย คัดเลือกจากกลุ่มประชากรที่มีอุบัติการณ์ MACE ร้อยละ 4.9 การขาดนัดนานกว่า 60 วันมีความสัมพันธ์กับ MACE ทั้งในการวิเคราะห์ตัวแปรเดียว (OR=8.66, 95% CI: 1.67–44.93, p=0.01) และพหุตัวแปร (Model 2: aOR=10.37, 95% CI: 1.75–61.60, p=0.01) โรคเบาหวานเป็นปัจจัยเสี่ยงอิสระ (aOR=7.08, 95% CI: 1.39–36.13, p=0.02) และ complete revascularization ภายใน 45 วันเป็นปัจจัยป้องกันในเฉพาะการวิเคราะห์ตัวแปรเดียว (OR=0.18, 95% CI: 0.05–0.67, p=0.01)
สรุป: การขาดนัดในระบบ Shared-Care มีความสัมพันธ์อย่างมีนัยสำคัญกับ MACE ภายใน 1 ปีหลัง STEMI ควรบูรณาการเข้าสู่ระบบแบ่งชั้นความเสี่ยงและพัฒนากลยุทธ์ติดตามเชิงรุกในโรงพยาบาลทุติยภูมิไทย
