Serum Tumor Markers in Gynecologic Cancers

Abstract

มะเร็งนรีเวชประกอบด้วย มะเร็งรังไข่ มะเร็งปากมดลูก มะเร็งเยื่อบุโพรงมดลูก มะเร็งปากช่องคลอด และมะเร็งเนื้อรก รวมถึงมะเร็งในวอวัยวะสืบพันธ์สตรี ได้แก่ มะเร็งช่องคลอด และมะเร็งท่อนำไข่ เป็นต้น


ปี พ.ศ. 2544 - 2546 ประเทศไทยพบผู้ป่วยมะเร็งนรีเวช จำนวน 28,370 คน คิดเป็นร้อยละ 23.8 ของมะเร็งที่พบในสตรีทั้งหมด(1) มะเร็งปากมดลูกเป็นมะเร็งนรีเวชกรรมที่พบมากเป็นอันดับที่หนึ่งของโลก และในประเทศไทย รองลงมาได้แก่มะเร็งรังไข่และมะเร็งเยื่อบุโพรงมดลูก ตามลำดับ มีความพยายามคิดค้นการคัดกรองโรคมะเร็งในระยะเริ่มต้น เพื่อเพิ่มอัตราการรอดชีพของผู้ป่วยในมะเร็งปากมดลูก การตรวจเซลล์ที่ปากมดลูก (Cervical cytologic screening) ได้รับการพิสูจน์แล้วว่าสามารถลดอุบัติการณ์โรคมะเร็งปากมดลูกได้หลายประเทศทั่วโลก มีผลให้อัตราการรอดชีพของผู้ป่วยมะเร็งปากมดลูกเพิ่มขึ้นอย่างมาก แต่ในมะเร็งอื่นๆทางนรีเวชกรรมยังไม่มีวิธรการตรวจคัดกรองดังกล่าว มีความพยายามคิดค้นสารชีวภาพเพื่อนำมาใช้ประโยชน์ในการตรวจมะเร็งในระยะเริ่มต้น ใช้ในการติดตามการรักษา และเฝ้าระวังการกลับมาเป็นซ้ำในโรคมะเร็งทางนรีเวชกรรม บทความนี้จะกล่าวถึงการใช้ serum tumor marker ทางมะเร็งนรีเวช โดยเน้นโรคที่พบบ่อย ได้แก่ มะเร็งรังไข่ มะเร็งปากมดลูก และมะเร็งเยื่อบุโพรงมดลูก
