3D prototyping for adjustment of pre-bended plates for mandibular defect reconstruction : A case report

Abstract
Facial deformity, owing to the expansion from cyst of the jaw, usually causes malocclusion and esthetic concern to the patients. The goal of the treatment is to eradicate the cause, as well as, correct facial asymmetry. Surgical removal requires enucleation of neoplasm and bone recontouring, which results in a compromised facial profile. Currently, with the advancement of technology, computer-aided design and computer-aided manufacturing (CAD-CAM) are utilized to assist in the reconstruction of the mandible. The 3D printer, a form of CAM, typically aids in surgical planning and pre-bending the titanium reconstruction plate.
A Thai male patient with a dentigerous cyst related to an embedded tooth in the mandible. Upon examination, the radiographic image showed alveolar bone destruction, leaving only 5 mm of mandibular bone height. A titanium plate was planned to reinforce the mandible and prevent further fractures. A 3D-printed model of the mandible was fabricated, and the bony defect was filled with self-polymerizing acrylic resin. The titanium plate was pre-bent before surgery. Determining the size and shape of the titanium plate before surgery helps reduce surgical time and ensures surgical precision. Patient reported that no complications occurred and that he can masticate normally.
