Associated factors of hypoglycemia in type 2 diabetes patient in Bangkhla hospital, Bangkhla district, Checheongsao province

Abstract
Background: Diabetes Mellitus is a common chronic disease which can be found in all regions worldwide and is also a threatening health issue in Thailand. Poorly-controlled patients have poor prognosis and are at high risk of life-threatening complications. One of concerning complications of diabetes mellitus is hypoglycemia. There are about 50 cases of hypoglycemia presented at the emergency department of Bangkhla Hospital in one year. This research studies associated factors of hypoglycemia in type 2 diabetes patients to raise the awareness of the disease and to develop more effective diabetes healthcare procedures. Methodology: The analytical crosssectional study was conducted by using secondary data from the database of type 2 diabetes patients at Bangkhla Hospital from May 2015 to April 2016, with a purpose of searching for associated factors of hypoglycemia in 1969 patient from the database above. The analysis is performed using a multiple logistic regression method and the result is presented in the prevalence rate ratio or odds ratio at confidence interval of 95% (p-value < 0.05) Results: Data were collected the database of 1969 patients at Bangkhla Hospital. It was found that there were 30 patients or 1.52 percent presented with hypoglycemia. The multiple logistic regression analysis using a univariable association method showed an association of female (p-value 0.03) and a creatinine level of more than 2.0 (p-value < 0.01), and when the factors were analyzed using a multivariable association, an association of female (OR: 3.17; 95%CI: 1.19-8.42) and a creatinine level of more than 2.0 (OR: 6.08; 95%CI: 2.38-15.50) was found. Conclusion and recommendation: This research studied associated factors of hypoglycemia in type 2 diabetes patients that were divided into two groups; patients presented with hypoglycemia and patients without hypoglycemia. It was found that there were 1.52 percent of patients presented with hypoglycemia. Although the number might be small but hypoglycemia is a life-threatening condition. Doctors and healthcare providers should pay special attention to diabetes patients with hypoglycemia. It was found that female patients and a creatinine level of more than 2.0 are associated factors of hypoglycemia, as previously reported. Doctors should carefully treat female patients with elevated creatinine levels.
