การพัฒนาการจัดการระบบการดูแลผู้ป่วยโรคเรื้อรังของ โรงพยาบาลส่งเสริมสุขภาพตำบล : กรณีศึกษา โรงพยาบาลส่งเสริมสุขภาพ ตำบลมะลวน อ.พุนพิน จ.สุราษฏร์ธานี

Abstract

บทคัดย่อ

          การวิจัยนี้เป็นการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม เพื่อศึกษาการดำเนินงานและผลการจัดระบบการดูแลผู้ป่วยโรคเรื้อรัง การวิจัยประกอบด้วย การวิเคราะห์ปัญหาและความต้องการ การวางแผน การดำเนินการ และสรุปบทเรียน เก็บข้อมูลโดยการสังเกต สอบถาม และประชุมเชิงปฏิบัติการ ใช้ระยะเวลา 1 ปี วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและการวิเคราะห์ข้อมูลเชิงเนื้อหา ผลการวิจัยสรุปได้ดังนี้

1. รพ.สต.มะลวน มีการจัดระบบการดูแลผู้ป่วยโรคเรื้อรังระดับมากที่สุด 4 ด้าน คือ การเตรียมข้อมูล การประเมินสภาพและคัดกรองผู้ป่วย การให้บริการต่อเนื่องฯและการเชื่อมต่อบริการ  ระดับปานกลาง คือ การจัดบริการด้านเภสัชกรรม การดำเนินการเชิงรุกร่วมกับชุมชน การให้คำปรึกษาและเสริมศักยภาพในการดูแลตนเองและการปรับพฤติกรรม

2.  การจัดทำแผนการพัฒนาระบบการดูแลผู้ป่วยเรื้อรังมีดังนี้  1) การพัฒนาระบบข้อมูลและจัดระบบการบันทึกให้เป็นปัจจุบัน ตลอดจนนำข้อมูลมาใช้วิเคราะห์เพื่อการดูแลในทุกมิติฯ 2) การปรับพฤติกรรมของประชาชน โดยการให้ความรู้เรื่องการรับประทานอาหารเพื่อลดโรค 3) การพัฒนาศักยภาพ อสม.ด้านการดูแลผู้ป่วยฯ พบว่า มีความรู้ มีทัศนคติที่ดีต่อการดูแลผู้ป่วยเบาหวาน ความดันโลหิตสูงในชุมชน สามารถประเมินอาการของกลุ่มเสี่ยงและสามารถให้คำแนะนำเบื้องต้นเรื่องการดูแลตนเองของกลุ่มป่วยได้ และ 4) การพัฒนาความร่วมมือกับภาคท้องถิ่นอยู่ระหว่างดำเนินการโดย รพ.สต.จะทำแผนความร่วมมือกับองค์กรปกครองส่วนท้องถิ่นในปีงบประมาณปีต่อไป

The Development of Chronic Disease Management for Patients in a Health Promotion Hospital:  A Case Study of Maluan Health Promotion Hospital, Phunphin District, Suratthani Province

Abstract

          This participatory action research (PAR) was conducted to investigate the process of chronic disease management for patients with diabetes mellitus (DM) and hypertension (HT), community participation in disease management, and strategies for the development of a disease management plan. Health care personnel, health volunteers, and people in the community participated in every stage of this 1 year PAR study, which included problem and needs assessing, planning, implementing, and lessons learned summarizing. The researchers collected data from observations, asking questions, and a workshop.

The study showed that of all 8 components in the chronic disease management of Maluan Health Promotion Hospital, 4 components (data preparation, assessment and patient screening, continuous healthcare services, and referral care between higher level hospitals and

Maluan Hospital) were managed and implemented at the highest level. For the other 4 components, pharmaceutical service, chronic disease active implementation with community and local administration office, counseling and health promotion in self-care, and behavior change, were implemented at a moderate level.

         Based on the current assessment and implementation, the following 4 plans were developed: 1) data management in providing holistic care to patients, 2) behavior changes to prevent HT and DM, 3) development of the capability of healthcare personnel and community leaders in prevention, screening, and provision of care for patients with DM and HT, and 4) promoting community cooperation to systematically manage chronic diseases. 

          The first 3 project plans were implemented, it was found that data on HT and DM were used to analyze for all dimensions of disease management; people in community had a higher level of selecting healthy foods; health volunteers had a knowledge level in HT and DM management higher than an indicated level. The activities in the 4^th plan were still in the development process and would be fully implemented and evaluated in the fiscal year of 2016.
