The Development of a Case Management Model for Pediatric Patients with Acute Lymphoblastic Leukemia at Udonthani Hospital

Abstract

Acute Lymphoblastic Leukemia (ALL) is the most common cancer disease in pediatric patients. its impact direct to the patients and family. This study was a research and development study, with objective to develop a care management model for pediatric patients with ALL.The researcher collected data by interview Study in 15 pediatric patients with ALL admitted at Pediatric ward 3, Udonthani Hospital, 15 caregiver and 13 providers. who were multidisciplinary care team relative with ALL patient care. Data were collected by interviewed and tape recorded.There were 4 steps : situation analyzed, developed case manager model, implemented case manager model for caring patients and evaluation. Data were collected from December 2017 to March 2018 and analyzed by descriptive statistic. Analyzed an interview record and grouping for a case management model.

The research findings are as follows: (1.) In an analyze situation period, there are 13

employees who were multidisciplinary care 15 pediatric ALL patients and 15 caregiver were given the data and suggestion about how to care the patients. The providers almost were female 92.30% mean average of age 35.38 year old and 2.90 years experience time for caring ALL patient. The pediatric ALL were male 66.70% age 4-15 years old and caregiver were female 80.00% and were mother 60.00% (2.) Case management Model was developed in January 2018 The development model consisted of 4 factors 2.1) The factor of the policy and strategy of the Instituted, 2.2) The factor of provider teams, 2.3) The factor of the patient and care givers. and 2.4) The factor of community, school or the other hospital. (3.) The development of a Case Management Model for Pediatric Patients with Acute Lymphoblastic Leukemia at Udonthani Hospital appropriated with the context of Udonthani Hospital was performed in February - March 2018 and (4) found that the model with appropriated score were 88.61%

Conclusion: To promote a case management for caring these patients lead in effectiveness of care.
