การรักษาถุงน้ำเมือกพื้นปากโดยวิธีมาร์ซูเพียลไลเซชัน: รายงานผู้ป่วย 1 ราย และการทบทวนวรรณกรรม

ถุงน้ำเมือกพื้นปาก คือ รอยโรคที่มีลักษณะบวมนูน มีน้ำเมือกอยู่ภายใน เกิดในตำแหน่งพื้นปาก โดยมีสาเหตุจากการที่ท่อต่อมน้ำลายใต้ลิ้นได้รับบาดเจ็บแล้วมีน้ำเมือกรั่วจากท่อต่อมน้ำลาย หรือเกิดจากการอุดตันในท่อต่อมน้ำลายใต้ลิ้น วิธีการรักษาถุงน้ำเมือกพื้นปากซึ่งพบอัตราการกลับมีอาการโรคต่ำที่สุด คือ การรักษาที่มีการตัดต่อมน้ำลายใต้ลิ้นออก แต่มีความเสี่ยงในการทำอันตรายต่อเนื้อเยื่อข้างเคียง วิธีมาร์ซูเพียลไลเซชันเป็นทางเลือกหนึ่งในการจัดการถุงน้ำเมือกพื้นปากที่ได้ผลดี สามารถรักษาภายใต้การให้ยาชาเฉพาะที่ได้ ทันตแพทย์ต้องพิจารณาแผนการรักษาที่เหมาะสมร่วมกับผู้ป่วยและผู้ปกครอง อธิบายข้อดี ข้อเสีย และภาวะแทรกซ้อนที่อาจเกิดขึ้นจากการรักษาในแต่ละวิธี รวมถึงพบการกลับมีอาการโรคได้ ดังนั้นจึงควรติดตามผลการรักษาอย่างต่อเนื่อง
บทความนี้เป็นรายงานผู้ป่วยหญิงไทยอายุ 12 ปี มาพบทันตแพทย์ด้วยอาการบวมบริเวณพื้นปากใต้ลิ้นด้านซ้าย จากการตรวจทางคลินิก บริเวณพื้นปากใต้ลิ้นด้านซ้าย พบอาการบวมนูนในลักษณะซึ่งมีของเหลวอยู่ภายใน สีโทนน้ำเงิน ให้การวินิจฉัยทางคลินิกว่าเป็นถุงน้ำเมือกพื้นปาก และให้การรักษาโดยการทำมาร์ซูเพียลไลเซชันภายใต้ยาชาเฉพาะที่ จากการติดตามผลการรักษา 32 เดือน พบว่าผู้ป่วยไม่มีการกลับมีอาการโรค
