ผลการรักษาและความปลอดภัยของการใช้ rtPA ทางหลอดเลือดดำสำหรับ Minor Ischemic Stroke: ประสบการณ์จากโรงพยาบาลอุดรธานี

การรักษาผู้ป่วยโรคหลอดเลือดสมองตีบเฉียบพลันด้วยยาละลายลิ่มเลือด recombinant tissue plasminogen activator (rtPA; alteplase) มีประสิทธิภาพในการลดความพิการและการเสียชีวิต อย่างไรก็ตามในผู้ป่วยที่มีอาการเล็กน้อย (minor stroke; NIHSS < 4) การให้ยา rtPA ยังคงมีข้อถกเถียง เนื่องจากต้องพิจารณาสมดุลระหว่างประโยชน์ด้านผลลัพธ์กับความเสี่ยงต่อภาวะเลือดออกในสมอง การศึกษานี้มีวัตถุประสงค์เพื่อประเมินผลลัพธ์ของการให้ยา rtPA เทียบกับกลุ่มที่ไม่ได้รับยาในผู้ป่วย minor stroke เพื่อเปรียบเทียบผลลัพธ์ทางคลินิกที่ประเมินความสามารถในการทำหน้าที่หรือใช้ชีวิตประจำวัน (functional outcome) โดยใช้คะแนน Modified Rankin Scale (mRS) ที่ 3 และ 6 เดือน และศึกษาการเกิดภาวะเลือดออกในสมองที่มีอาการ (symptomatic intracranial hemorrhage; sICH) ภายใน 6 เดือน เป็นการศึกษาแบบ retrospective cohort รวบรวมข้อมูลผู้ป่วยโรคหลอดเลือดสมองตีบที่มีคะแนน NIHSS < 4 เข้ารับการรักษาที่โรงพยาบาลอุดรธานี ระหว่างเดือนมกราคม–ธันวาคม 2567 เปรียบเทียบกลุ่มที่ได้รับยา rtPA กับกลุ่มที่ไม่ได้รับยา วิเคราะห์ข้อมูลโดยใช้สถิติ chi-square test, multivariable logistic regression สำหรับปรับปัจจัยกวน (Baseline NIHSS, Baseline mRS)
ผลการศึกษา: ผู้ป่วยทั้งหมด 124 ราย ได้รับยา rtPA 45 ราย (ร้อยละ 36.29) และไม่ได้รับยา 79 ราย (ร้อยละ 63.71) พบว่าที่ 3 เดือนหลังรักษา กลุ่มที่ได้รับ rtPA มีสัดส่วนของผลลัพธ์ดี (mRS 0–2) สูงกว่ากลุ่มที่ไม่ได้รับยา ( Odds ratio 4.39, 95% CI 1.56-12.40; p=0.003) ที่ 6 เดือนไม่พบความแตกต่างในกลุ่มที่ได้และไม่ได้ rtPA (Odds ratio 2.51, 95% CI 0.67-9.41; p =0.16) พบภาวะเลือดออกในสมองเพียง 1 ราย (ร้อยละ 2.2) ในกลุ่มที่ได้รับยา rtPA
สรุปผล: การให้ยา rtPA ในผู้ป่วย minor stroke มีความสัมพันธ์กับผลลัพธ์ทางคลินิกที่ประเมินความสามารถในการทำหน้าที่หรือใช้ชีวิตประจำวันดีขึ้นในระยะ 3 เดือน
