รายงานผู้ป่วย: การติดเชื้อรา Talaromycosis ที่แสดงอาการด้วยผื่น Umbilicated Papules ในผู้ป่วยเอดส์บริบทโรงพยาบาลชุมชน

Disseminated Talaromycosis เป็นการติดเชื้อราที่พบได้บ่อยในผู้ป่วยที่มีภาวะภูมิคุ้มกันบกพร่องโดยเฉพาะผู้ติดเชื้อเอชไอวีในภูมิภาคเอเชียตะวันออกเฉียงใต้ เกิดจากเชื้อ Talaromyces marneffei การวินิจฉัยล่าช้าอาจนำไปสู่ภาวะแทรกซ้อนและอัตราการเสียชีวิตที่สูงขึ้น โดยเฉพาะในบริบทโรงพยาบาลชุมชนที่มีทรัพยากรการวินิจฉัยจำกัด การพบผื่นตุ่มนูนมีรอยบุ๋มตรงกลางซึ่งเป็น pathognomonic finding ที่สำคัญร่วมกับการตรวจขูดผิวหนังและย้อม Wright stain เป็นเครื่องมือวินิจฉัยเบื้องต้นที่รวดเร็วและเข้าถึงได้ในบริบทโรงพยาบาลชุมชน รายงานผู้ป่วยรายนี้จึงจัดทำขึ้นเพื่อนำเสนอกรณีผู้ป่วยเอชไอวีที่มีผื่นลักษณะดังกล่าว และได้รับการวินิจฉัย Disseminated Talaromycosis โดยอาศัยการตรวจขูดผิวหนังและย้อม Wright stain เป็นเครื่องมือวินิจฉัยแยกโรคเบื้องต้นในบริบทโรงพยาบาลชุมชน
ผู้ป่วยหญิงไทยอายุ 30 ปี ผู้ติดเชื้อเอชไอวีระยะเอดส์ที่มีประวัติขาดยาต้านไวรัส TLD (Tenofovir/Lamivudine/ Dolutegravir) นานกว่า 3 ปี เข้ารับการรักษาที่โรงพยาบาลพระอาจารย์ฝั้นอาจาโร ด้วยอาการไข้ต่ำๆ น้ำหนักลด และ ผื่นลักษณะตุ่มนูนมีรอยบุ๋มตรงกลางที่ใบหน้า แขนขาและลำตัว จำนวนเซลล์ CD4 แรกรับ 88 เซลล์ต่อไมโครลิตร (ร้อยละ 12.2) การขูดผิวหนังและย้อม Wright stain ตรวจพบเชื้อราแบบ binary fission yeast และการเพาะเชื้อราในเลือดตรวจพบเชื้อ Talaromyces marneffei นอกจากนี้ผลเพาะเชื้อแบคทีเรียในเลือดจาก 2 specimens ตรวจพบ Salmonella typhi ยืนยันภาวะการติดเชื้อร่วม ผู้ป่วยได้รับการรักษา Salmonella typhi bacteremia ด้วย Ceftriaxone 2 กรัม ทางหลอดเลือดดำวันละ 1 ครั้ง เป็นเวลา 2 สัปดาห์ สำหรับ Disseminated Talaromycosis ให้การรักษาโดยใช้ Amphotericin B ขนาด 0.8 มิลลิกรัมต่อกิโลกรัม เป็นเวลา 2 สัปดาห์ ตามด้วย Itraconazole 200 มิลลิกรัม ทุก 8 ชั่วโมง เป็นเวลา 3 วัน ตามด้วย Itraconazole 200 มิลลิกรัม ทุก 12 ชั่วโมง เป็นเวลา 10 สัปดาห์ และ Itraconazole 200 มิลลิกรัมต่อวัน จนผู้ป่วยมีเซลล์ CD4 มากกว่า 100 เซลล์ต่อไมโครลิตร ผู้ป่วยได้รับ Cotrimoxazole (Trimethoprim 80 มิลลิกรัม/Sulfamethoxazole 400 มิลลิกรัม) 2 เม็ดวันละ 1 ครั้ง เพื่อป้องกันการติดเชื้อฉวยโอกาสจนผู้ป่วยมีเซลล์ CD4 มากกว่า 200 เซลล์ต่อไมโครลิตร ทั้งนี้ได้เริ่มยาต้านไวรัสใหม่หลังจากได้รับ Amphotericin B ครบ 2 สัปดาห์ ผู้ป่วยตอบสนองดีต่อการรักษาและหลังจำหน่ายออกจากโรงพยาบาลมีการติดตามผื่นผิวหนังที่ 3 เดือน พบว่าผื่นผิวหนังดีขึ้นอย่างมีนัยสำคัญ
สรุปผลการศึกษา: ในผู้ป่วยเอชไอวีที่มีภาวะภูมิคุ้มกันบกพร่องรุนแรงและมาด้วยผื่นตุ่มนูนมีรอยบุ๋มตรงกลาง การตรวจขูดรอยโรคผิวหนังและย้อม Wright stain สามารถช่วยสนับสนุนการวินิจฉัย talaromycosis ได้อย่างรวดเร็วในบริบทโรงพยาบาลชุมชน ขณะที่การเพาะเชื้อรายังคงมีบทบาทในการยืนยันการวินิจฉัย กรณีนี้ยังแสดงให้เห็นความสำคัญของการพิจารณาการติดเชื้อร่วมและการส่งเพาะเชื้อในเลือดทั้งแบคทีเรียและเชื้อราเมื่อมีข้อบ่งชี้
