Functional Outcomes, Implant Survivorship, and Complications Following Primary Total Hip Arthroplasty in Octogenarians: A Systematic Review and Meta-Analysis

Abstract

This study aimed to evaluate functional outcomes, implant survivorship, and complications following primary total hip arthroplasty (THA) in octogenarians. A systematic review and meta-analysis were conducted following PRISMA 2020 guidelines. CINAHL, Embase, MEDLINE, and Scopus databases were searched through March 2025. Ten studies comprising 8,027 patients were included. The pooled mean Harris Hip Score improvement was 38.1 points (95% CI: 33.2-43.1), exceeding the minimal clinically important difference. Implant survivorship reached 96.5% at 5 years and 93.2% at 10 years. Surgical complications included dislocation (2.8%) and surgical site infection (1.2%), while medical complications occurred in 14.5% of patients. Primary THA in octogenarians yields clinically meaningful functional improvement and reliable implant longevity with appropriate patient selection and perioperative management.
