การพัฒนารูปแบบการบันทึกคุณภาพสาเหตุการตายนอกสถานบริการของจังหวัดปัตตานี ปี 2560 - 2562

บทคัดย่อ
การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อ 1) พัฒนารูปแบบการบันทึกคุณภาพสาเหตุการตาย 2) ศึกษาผลการพัฒนารูปแบบ การวิจัยและพัฒนาครั้งนี้แบ่งเป็น 4 ขั้นตอน (1) วิเคราะห์สถานการณ์ (2) พัฒนาร่างรูปแบบ (3) ทดลองใช้ร่างรูปแบบ ประเมินและปรับร่าง (4) นำรูปแบบใช้จริง เก็บข้อมูลด้วยแบบสอบถามและแบบบันทึกตรวจสอบคุณภาพข้อมูลการตาย และสนทนากลุ่ม ผู้ให้ข้อมูล คือ เจ้าหน้าที่งานทะเบียนและบัตร และเจ้าหน้าที่สาธารณสุขระดับอำเภอและระดับตำบล วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ผลการศึกษา พบว่า รูปแบบที่พัฒนา โดยเพิ่มขั้นตอนประวัติการเจ็บป่วยของผู้ตายและสันนิษฐานสาเหตุการตายหลังใบรับแจ้งการตายระบบตรวจสอบคุณภาพสาเหตุการตายโดยทีมจากทั้งสองหน่วยงาน และระบบประสานงานเพื่อให้คำปรึกษาจากการพัฒนารูปแบบ พบว่า สัดส่วนของสาเหตุการตายไม่ชัดแจ้งลดลงจากร้อยละ 37.04 เป็น ร้อยละ 8.90 ปัจจัยที่สำคัญ คือ การสร้างค่านิยมร่วมกัน (Shared values) โดยให้ความสำคัญกับการบันทึกสาเหตุการตายที่มีคุณภาพ การปรับระบบงาน (System) โดยแลกเปลี่ยนประสบการณ์ผู้ปฏิบัติ และการพัฒนาทักษะ (Skill) ผู้เกี่ยวข้อง โดยการอบรมและเรียนรู้จากการตรวจสอบคุณภาพข้อมูลการตายในระดับอำเภอร่วมกัน
