ปัจจัยเสี่ยงของมะเร็งปากมดลูกและการเปรียบเทียบการตรวจคัดกรองมะเร็งปากมดลูกโดย 2 วิธี

บทคัดย่อ
งานวิจัยนี้มีวัตถุประสงค์เพื่อหาปัจจัยเสี่ยงของมะเร็งปากมดลูกและเปรียบเทียบการตรวจคัดกรองมะเร็งปากมดลูกด้วยวิธี Conventional Pap Smear หรือ CPS (กลุ่มที่ 1) และวิธี HPV DNA test ร่วมกับ Liquid-based cytology หรือ LBC (กลุ่มที่ 2) จากสตรีที่รับการตรวจคัดกรองมะเร็งปากมดลูกในโรงพยาบาลสมเด็จพระเจ้าตากสินมหาราช จำนวน 384 คน แบ่งเป็นกลุ่มที่ 1 และ 2 จำนวน 207 คน และ 177 คน ตามลำดับ ตามความสมัครใจของผู้รับการตรวจ จากสถิติ Fisher’s exact test พบว่า สุรามีความสัมพันธ์กับผลบวกจากการตรวจคัดกรองมะเร็งปากมดลูกเหมือนกันทั้ง 2 กลุ่ม จากสถิติ Mantel-Haenszel test การตรวจคัดกรองกลุ่มที่ 2 พบผลเป็นบวก 6.79 เท่า (OR = 6.79, 95%CI: 1.49-31.07) ของกลุ่มที่ 1 การตรวจคัดกรองด้วยวิธี HPV DNA test ร่วมกับ LBC มีแนวโน้มที่จะมีความไวมากกว่าวิธี CPS สตรีที่ดื่มสุราควรได้รับการป้องกันการเป็นมะเร็งปากมดลูก และควรมีการวิจัยซ้ำในกลุ่มตัวอย่างที่มีขนาดใหญ่
