บทคัดย่อ
การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่สัมพันธ์กับภาวะแทรกซ้อนทางไตของผู้ป่วยโรคเบาหวานและความดันโลหิตสูง ณ โรงพยาบาลโขงเจียม จังหวัดอุบลราชธานี กลุ่มตัวอย่างคือผู้ป่วย 350 ราย สุ่มแบบหลายขั้นตอน เครื่องมือวิจัยเป็นแบบสอบถามความรู้ การรับรู้ส่วนบุคคล แรงสนับสนุนทางสังคมและพฤติกรรมการป้องกัน มีค่าความเชื่อมั่นระหว่าง 0.71-0.85 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการถดถอยโลจิสติกพหุคูณ นำเสนอผลด้วย Adjusted Odds Ratio (ORadj) ช่วงความเชื่อมั่นที่ 95% กำหนดระดับนัยสำคัญที่ 0.05 ผลการวิจัยพบว่า 26% ของผู้ป่วยมีภาวะแทรกซ้อนทางไต โดย 50.6% มีระดับการรับรู้ส่วนบุคคลระดับมาก 81.7% รับรู้ความรุนแรงของภาวะแทรกซ้อนระดับมากที่สุด และ 34% รับรู้อุปสรรคในการป้องกันระดับน้อยที่สุด นอกจากนี้ 80.9% ได้รับแรงสนับสนุนทางสังคมระดับมาก และ 68.6% มีพฤติกรรมการป้องกัน ระดับดี อย่างไรก็ตาม พบว่าพฤติกรรมที่ควรปรับปรุงคือการรับประทานอาหาร (35.4%) ปัจจัยที ่สัมพันธ์กับภาวะแทรกซ้อนทางไตอย่างมีนัยสำคัญทางสถิติ ได้แก่ เพศ (ORadj = 0.13, 95%CI: 0.03-0.59) อายุ (ORadj = 0.15, 95%CI: 0.03-0.71) รายได้ต่อเดือน (ORadj = 0.56, 95%CI: 0.12-0.56) ระยะเวลาที่ได้รับการวินิจฉัยว่าเป็นโรค (ORadj = 8.81, 95%CI: 2.54-30.53) และพฤติกรรมการป้องกันภาวะแทรกซ้อนทางไต (ORadj = 0.02, 95%CI: 0.00-0.07) ผลวิจัยนี้สามารถนำไปประยุกต์ใช้ในคลินิกโรคไม่ติดต่อเรื้อรังของสถานพยาบาล ที่มีบริบทใกล้เคียง เพื่อป้องกันและชะลอการเสื่อมของไตในระยะยาว
