การพัฒนาระบบบูรณาการการดูแลต่อเนื่องที่บ้านสำหรับผู้ป่วยโรคหลอดเลือดสมองในเครือข่ายสุขภาพอำเภอพรหมคีรี จังหวัดนครศรีธรรมราช

บทคัดย่อ

การวิจัยและพัฒนานี้มีวัตถุประสงค์เพื่อพัฒนาและประเมินประสิทธิผลของระบบบูรณาการการดูแลต่อเนื่องที่บ้านสำหรับผู้ป่วยโรคหลอดเลือดสมองในเครือข่ายสุขภาพอำเภอพรหมคีรี จังหวัดนครศรีธรรมราช ดำเนินการ 4 ระยะ ได้แก่ 1) ศึกษาบริบทและวิเคราะห์ปัญหา 2) พัฒนารูปแบบการดูแลแบบมีส่วนร่วมภายใต้ PHROM KHIRI Model โดยประยุกต์ใช้แนวคิดแรงสนับสนุนทางสังคมของ House 3) ทดลองใช้รูปแบบด้วยการวิจัยกึ่งทดลองแบบกลุ่มเดียววัดก่อนและหลัง เป็นเวลา 20 สัปดาห์ ในผู้ป่วยโรคหลอดเลือดสมองและผู้ดูแลหลัก 38 คู่ และ 4) ประเมินผล ปรับปรุง และขยายผล เก็บข้อมูลด้วยการสัมภาษณ์และสนทนากลุ่ม และแบบประเมินการดูแลผู้ป่วย การสนับสนุนจากชุมชน คุณภาพชีวิต และความพึงพอใจ วิเคราะห์ข้อมูลด้วยสถิติพรรณนา Paired t-test และการวิเคราะห์เชิงเนื้อหา ผลการวิจัยพบว่าหลังทดลองใช้ PHROM KHIRI Model คะแนนเฉลี่ยการดูแลผู้ป่วยของผู้ดูแลการสนับสนุนจากชุมชน และคุณภาพชีวิตของผู้ป่วยเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ โดยมีผลต่างค่าเฉลี่ย 0.28 คะแนน (95% CI: 0.21–0.35), 0.63 คะแนน (95% CI: 0.47–0.79) และ 0.55 คะแนน (95% CI: 0.42–0.68) ตามลำดับ ผู้เกี่ยวข้องมีความพึงพอใจต่อรูปแบบที่พัฒนาขึ้นโดยรวมในระดับดีมากร้อยละ 81.43 สรุปได้ว่า PHROM KHIRI Model ช่วยจัดระบบการดูแลต่อเนื่องที่บ้านให้เชื่อมโยงระหว่างหน่วยบริการครอบครัว และชุมชน ส่งผลให้การดูแลของผู้ดูแลการสนับสนุนทางสังคมและคุณภาพชีวิตของผู้ป่วยดีขึ้น
