บทคัดย่อ

ภูมิหลัง: ปัญหาสตรีติดเชื้อซิฟิลิสขณะตั้งครรภ์ รวมทั้งทารกที่คลอดแล้วเกิดภาวะแทรกซ้อนจากมารดาที่ติดเชื้อซิฟิลิสนั้น มีหลายการศึกษารายงานพบแนวโน้มที่เพิ่มมากขึ้นในทุก ๆ ปีแต่ผลกระทบของการติดเชื้อซิฟิลิสต่อผลลัพธ์ของการตั้งครรภ์ในประเทศไทยยังคงมีการศึกษาน้อยมากวัตถุประสงค์: เพื่อศึกษาความชุก และปัจจัยที่มีความสัมพันธ์กับภาวะแทรกซ้อนต่อการตั้งครรภ์ในสตรีที่ติดเชื้อซิฟิลิสวิธีการ: เป็นการศึกษาเชิงพรรณนา(retrospective descriptive study) ในสตรีที่ติดเชื้อซิฟิลิสขณะ ตั้งครรภ์ ที่มาคลอดในโรงพยาบาลกระทุ่มแบน จังหวัดสมุทรสาครตั้งแต่  1 ตุลาคม พ.ศ. 2561 ถึง 30 กันยายน พ.ศ. 2563 โดยเก็บข้อมูลย้อนหลังจากเวชระเบียน วิเคราะห์ข้อมูลด้วยสถิติพรรณนาและสถิติเชิงวิเคราะห์ผล: พบสตรีตั้งครรภ์ติดเชื้อซิฟิลิสที่มารับการรักษาจำนวน 84 ราย อายุเฉลี่ยของมารดาคือ 22.12 ± 5.53 ปีความชุกของมารดาที่ติดเชื้อซิฟิลิสในขณะตั้งครรภ์ร้อยละ 0.78ภาวะแทรกซ้อนที่พบคือ การคลอดก่อนกำหนดและทารกน้ำหนักตัวน้อย พบเท่ากันที่ 8 คน (ร้อยละ 9.5) ทารกตายในครรภ์ พบ 6 คน(ร้อยละ 7.1) ซิฟิลิสแต่กำเนิดและ ซิฟิลิสระบบประสาท พบเท่ากันคือ 4 คน (ร้อยละ 4.8) ทารกส่วนใหญ่ 50 คน (ร้อยละ 59.5) ไม่พบภาวะแทรกซ้อนจากการตั้งครรภ์ ปัจจัยที่มีความสัมพันธ์ต่อการเกิดภาวะแทรกซ้อนจากการตั้งครรภ์ในทารก ได้แก่ การฝากครรภ์ไม่ครบตามเกณฑ์ ( OR 5.64; 95% CI = 1.39, 22.73) VDRL titer ที่ตรวจพบครั้งแรกมากกว่าหรือเท่ากับ 1:8 (OR 2.63 ; 95% CI=1.06, 6.49) ปัจจัยที่ช่วยลดภาวะแทรกซ้อน ได้แก่ การเริ่มรักษาซิฟิลิสตั้งแต่ไตรมาสแรกของการตั้งครรภ์ (OR 0.25; 95% CI=0.06, 0.99) และการได้รักษาซิฟิลิสครบตามแนวทางการรักษา(OR 0.16 ; 95% CI= 0.04, 0.66)สรุป : การจัดระบบบริการฝากครรภ์ที่ได้มาตรฐาน หญิงตั้งครรภ์เข้าถึงการฝากครรภ์ในไตรมาสแรกได้รับการตรวจคัดกรองซิฟิลิสในระหว่างการตั้งครรภ์ ให้การรักษามารดาทันทีหากตรวจพบ จะช่วยลดอุบัติการณ์ของการเกิดโรคในสตรีตั้งครรภ์ และภาวะแทรกซ้อนในทารก
