การพัฒนารูปแบบการจัดการพฤติกรรมก้าวร้าวของผู้ป่วยจิตเวชในชุมชน โดยใช้การแก้ปัญหาอย่างสร้างสรรค์

การวิจัยครั้งนี้เพื่อพัฒนาและประเมิน รูปแบบการจัดการพฤติกรรมก้าวร้าวของผู้ป่วย จิตเวชในชุมชนโดยการแก้ปัญหาอย่างสร้างสรรค์ กลุ่มตัวอย่างคือ 1) บุคลากรสาธารณสุข 2) ผู้ป่วย จิตเวช 3) ผู้ดูแลผู้ป่วยจิตเวช และ 4) แกนนำ ชุมชน เครื่องมือที่ใช้ในการรวบรวมข้อมูล ได้แก่ 1) แบบสอบถามความคิดเห็น ปัญหาและอุปสรรค ในการจัดการพฤติกรรมก้าวร้าว 2) แผนการประชุม กลุ่มตามแนวคิดการแก้ปัญหาอย่างสร้างสรรค์ ข้อมูลเชิงคุณภาพที่ได้มาวิเคราะห์ด้วย การ วิเคราะห์เนื้อหา ส่วนข้อมูลเชิงปริมาณวิเคราะห์ โดยสถิติเชิงพรรณนา

ผลการวิจัยที่สำคัญ มีดังนี้ 1) รูปแบบการจัดการพฤติกรรมก้าวร้าวของผู้ป่วยจิตเวชในชุมชนโดยใช้การแก้ปัญหาอย่างสร้างสรรค์ ประกอบด้วย การร่วมมือระหว่างบุคคลากรสาธารณสุขและผู้มีส่วนได้ส่วนเสียในชุมชน โดย ร่วมกันค้นหาสาเหตุของพฤติกรรมก้าวร้าวของ ผู้ป่วยจิตเวช การสร้างเกณฑ์พิจารณาเลือกวิธี แก้ปัญหา และการสร้างแนวปฏิบัติการจัดการ พฤติกรรมก้าวร้าว 2) การนำรูปแบบการจัดการ พฤติกรรมก้าวร้าวของผู้ป่วยจิตเวชฯ ไปใช้ พบว่า บุคลากรสาธารณสุข ผู้ดูแลผู้ป่วยจิตเวช และแกนนำชุมชน สามารถจัดการกับผู้ป่วยจิตเวชที่มี พฤติกรรมก้าวร้าวรุนแรงได้อย่างมีประสิทธิภาพ ภาพรวมของความพึงพอใจในการนำรูปแบบการ จัดการฯ ไปใช้ ในระดับมาก 3) การบรรลุเป้าหมาย คือ มีทีมให้การช่วยเหลือ จำนวนผู้ป่วยทั้งหมด ได้รับการส่งต่อไปยังโรงพยาบาลพระศรีมหาโพธิ์ และไม่มีบุคคลอื่นในชุมชนได้รับบาดเจ็บจาก ผู้ป่วยจิตเวชที่มีพฤติกรรมก้าวร้าว

จากการวิจัยพบว่า รูปแบบการจัดการ พฤติกรรมก้าวร้าวของผู้ป่วยจิตเวชในชุมชนโดย การแก้ปัญหาอย่างสร้างสรรค์อยู่ในระดับพึงพอใจ แต่อย่างไรก็ตามในการปฏิบัติงานในชุมชน ควร ตระหนักในการเสริมสร้างความรู้ และทักษะ การประเมินพฤติกรรมก้าวร้าวของผู้ป่วยจิตเวช เพื่อช่วยในการพัฒนาการดูแลของชุมชนให้มี ประสิทธิภาพ

Abstract

The purpose of developmental research was to develop and evaluate the model of managing aggressive behavior of persons with mental illness in the community using creative problem solving. Health personnel, persons with mental illness, caregivers and key-person in community were selected as research participant. Research instruments consisted of 1) the questions of managing aggressive behaviors and their obstacles, and 2) the group discussion plan based on creative problem solving. Qualitative data were analyzed using content analysis and quantitative data were analyzed using descriptive statistics. Major findings were as follows: 1) the managing procedure aggressive behavior of persons with mental illness in the community using creative problem solving consisted of the collaboration between health personnel and key-person in the community by identifying causes of aggressive behaviors, developing criteria of problem solving and developing the guideline of managing aggressive behaviors, 2) For implementation, health personnel, caregivers and key-person in community effectively dealt with aggressive behavior of persons with mental illness and also demonstrated all satisfaction scale in good level and 3) For achieved goal, persons with serious mental illness received a quality care with community team and referred toward Prasrimahabhodi Psychiatric Hospital. In addition, harmfulness toward patients and other people in the community was not found. The result noted that satisfaction of implementation the model of managing aggressive behavior of persons with mental illness in the community using creative problem solving was acceptable. However, for approaching in the community, it should provide knowledge and skills of aggressive behavior appraisal for effectively improving a community care.
