แผนการจัดการดูแลทางคลินิกสำหรับผู้ป่วยที่ได้รับการผ่าตัดเปลี่ยนข้อเข่าเทียมนี้ ได้รับการพัฒนาโดยทีมสหสาขาวิชาชีพจากหลักฐานเชิงประจักษ์และมาตรฐานในการดูแลรักษา ตั้งแต่แรกรับจนกระทั่งจำหน่ายกลับบ้าน ซึ่งเป็นการรวบรวมแผนการจัดการดูแลและนำสู่การ ปฏิบัติ จากการประเมินผลการนำแผนการจัดการดูแลทางคลินิกสู่การปฏิบัติ พบว่าสามารถลด ระยะเวลานอนโรงพยาบาล ลดความเสี่ยงต่อการเกิดภาวะแทรกซ้อนและลดอัตราการกลับเข้ารับ การรักษาซ้ำโดยไม่ได้คาดหมาย ปัจจัยแห่งความสำเร็จคือ การปรับเปลี่ยนระบบการทำงาน ความร่วมมือ ของทีมสหสาขาวิชาชีพ การกำหนดเป้าหมายและตัวชี้วัดที่ชัดเจน ตลอดจนการพัฒนาอย่างต่อเนื่อง และการติดตามประเมินผล

Abstract

Clinical pathway for total knee arthroplasty patients was developed using evidenced-based and standardized protocols by a multidisciplinary team to care for patients from admission to discharge from the hospital. The objective of implementation of this clinical pathway for total knee arthroplasty was to assemble the standard of care and then apply to practice. From the evaluation of the implementation of the clinical pathway, it was found that the length of hospital stay, risks for complications, re-admission rate and cost reduced without adverse effects on clinical outcomes. The successful factors include the cooperation of multidisciplinary members, clear goal setting and clinical indicators, continuous improvement, as well as effective monitoring and evaluation.
