Relapsing polychondritis

A 47-year-old Thai man presented with fever, headache and conjunctivitis for 1 week. He was admitted and treated as leptospirosis with intravenous ceftriaxone and oral doxycycline for 12 days but his pyrexia and conjunctivitis continued. After 1 week of admission, he developed bilateral erythema, edema and tenderness of the ears that spared his earlobes which were consistent with auricular chondritis and developed concomitant migratory polyarthritis. He also had 1.5 year history of recurrent inflammation of both eyes and self-limited left earache and hearing loss 1 year ago. A clinical diagnosis of relapsing polychondritis was made and prednisolone 30 mg/day was begun with the clinical improvement in 2 days.
