การศึกษารูปแบบบริการการให้ยาป้องกันการติดเชื้อเอชไอวีก่อนการสัมผัสเชื้อ ประเทศไทย

การศึกษาครั้งนี้เป็นการศึกษาเชิงพรรณนาเพื่อศึกษารูปแบบการจัดบริการยาป้องกันการติดเชื้อเอชไอวีก่อนการสัมผัสเชื้อ (pre-exposure-prophylaxis, PrEP) และเพื่อศึกษาปัญหาอุปสรรคและข้อเสนอแนะการจัดบริการ PrEP ในประเทศไทย กลุ่มตัวอย่าง คือ บุคลากรที่ปฏิบัติหน้าที่ในหน่วยบริการนำร่องจัดบริการ PrEP ในระบบหลักประกันสุขภาพแห่งชาติ และมีความสมัครใจในการให้ข้อมูล จำนวน 129 คน จากหน่วยบริการ 110 แห่ง เก็บรวบรวมข้อมูลด้วยแบบสำรวจ และแบบสัมภาษณ์ทีมสหวิชาชีพหน่วยบริการสุขภาพ 12 แห่ง วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา และวิเคราะห์ข้อมูลเชิงคุณภาพแบบการจัดกลุ่มข้อมูล ผลการศึกษา พบว่า การจัดบริการ PrEP แบ่งเป็น 3 รูปแบบ ได้แก่ (1) หน่วยบริการเป็นผู้จัดบริการ (2) หน่วยบริการรับส่งต่อจากองค์กรภาคประชาสังคมหรือแกนนำชุมชนหรืออื่นๆ และ (3) หน่วยบริการร่วมจัดบริการกับองค์กรภาคประชาสังคม โดยมีการจัดบริการ PrEP ที่คลินิกยาต้านไวรัสเอชไอวีมากที่สุด รองลงมา คือ คลินิกให้บริการปรึกษา และคลินิกเฉพาะที่ให้บริการ PrEP คิดเป็นร้อยละ 81.40, 45.74 และ 34.88 ตามลำดับ กลุ่มเป้าหมายที่รับบริการเป็นกลุ่มชายมีเพศสัมพันธ์กับชายมากที่สุด รองลงมา คือ กลุ่มคู่ผลเลือดต่าง และกลุ่มหญิงข้ามเพศ คิดเป็นร้อยละ 17.32, 16.24 และ 14.34 ตามลำดับ และการค้นหากลุ่มเป้าหมายนำเข้าสู่บริการ PrEP ส่วนใหญ่มาจากบริการชวนคู่ของผู้ติดเชื้อเอชไอวีมาตรวจเอชไอวี รองลงมา มาจากการประสานและส่งต่อกับจุดอื่นๆ ในหน่วยบริการ และจากการทำงานเชิงรุกร่วมกับองค์กรภาคประชาสังคม คิดเป็นร้อยละ 80.62, 74.42 และ 46.51 ตามลำดับ ทว่ายังมีข้อจำกัดในการพัฒนารูปแบบวิธีการเข้าถึงกลุ่มเป้าหมาย เรื่องอัตรากำลัง ช่องทางการเข้าถึงบริการ และการตีตราและเลือกปฏิบัติ ผู้ให้บริการบางส่วนยังมีความกังวลและไม่มั่นใจในการให้บริการ โดยมีข้อเสนอเพิ่มหน่วยบริการ PrEP สร้างความร่วมมือประสานส่งต่อและร่วมจัดบริการกับองค์กรภาคประชาสังคมและหน่วยบริการในชุมชน เร่งสร้างความเข้าใจในกลุ่มบุคลากรถึงความสำคัญในการจัดบริการ ประชาสัมพันธ์สร้างความเข้าใจที่ถูกต้องเกี่ยวกับยา PrEP ที่สามารถเลือกใช้ร่วมไปกับวิธีการป้องกันอื่นๆ อย่างเหมาะสม
