Primary Health Care Service Provision for Diabetes Mellitus and Hypertension Patients of Primary Care Units in Khonkaen Province

Abstract

This qualitative research (case study) aimed to explore process and supporting system of primary health care (PHC) services for diabetes mellitus (DM) and hypertension (HT) patients provided in Muang, Nampong and Ubolratana districts, Khonkaen province. Samples were purposive with 170 health personnel, providing PHC services for DM and HT patients and working at PHC units: Primary Care Clusters (PCCs), covering pilot and general PCCs, NCD clinics of community hospitals, and also Tambon health promoting hospitals in Muang, Nampong and Ubolratana districts (pilot areas in Thailand). Data were collected by focus groups and analyzed by content analysis. The results revealed that PHC units in these districts illustrated the provision process and the supporting system quite the same way. Regarding the provision process, Clinic NCD of Khonkaen and Ubolratana hospitals seemed to be more remarkable than others in various aspects. However, in the aspects of continuity of care and coordination, and also share care plan, the pilot PCCs of Muang and Ubolratana districts seemed to be more outstanding than others respectively. In Nampong district, particularly, the pilot PCC seemed to be more notable than others in all aspects. For supporting system, in Muang district, clinic NCD of Khonkaen hospital seemed to be more outstanding than others, while in Nampong and Ubolratana districts, the pilot PCCs seemed to be more remarkable than others, except in the aspect of standard service development that the pilot PCC of Ubolratana district seemed intermittent. It seems having opportunities for developing PHC services in the pilot PCCs. Therefore, policy and practice should be more supported by Ministry of Public Health and others for further development of PCC.
