บทคัดย่อ
หญิงตั้งครรภ์มีความจำเป็นต้องใช้ยาระงับปวดกลุ่ม
โอปิออยด์ (opioids) สำหรับระงับอาการปวดเฉียบพลันและ
เรื้อรังต่างๆ เป็นที่ยอมรับกันว่ายาโอปิออยด์ในกระแสเลือด
ของหญิงตั้งครรภ์ผ่านจากมารดาไปยังทารกในครรภ์
การศึกษาแบบย้อนหลังสรุปว่าการใช้ยากลุ่มโอปิออยด์
บางชนิดในไตรมาสแรกของการตั้งครรภ์สัมพันธ์กับการเกิด
neural tube defect, gastroschisis และหัวใจพิการแต่
กำเนิดของทารกในครรภ์ การใช้ยากลุ่มโอปิออยด์เป็นเวลา
นานในไตรมาสสุดท้ายทำให้ทารกที่คลอดออกมามีภาวะ
neonatal abstinence syndrome (NAS) คือภาวะที่
ทารกแรกคลอดมีอาการถอนยา (withdrawal symptom) ได้
จุดประสงค์ของบทความฟื้นวิชานี้คือรวบรวมผลการ
ศึกษาที่เกี่ยวข้องกับการใช้ยากลุ่มโอปิออยด์ในหญิงตั้งครรภ์
รวมทั้งการเกิดความพิการในทารกและภาวะที่ทารกแรก
คลอดมอี าการถอนยา ดงั นั้นการใชย้ ากล่มุ นี้ในหญิงตั้งครรภ์
จึงเน้นให้ยาโอปิออยด์ในขนาดที่น้อยที่สุดที่ระงับปวดได้มี
ประสิทธิภาพ และเฉพาะในกรณีที่พิจารณาแล้วว่ามีความ
จำเป็นที่เกิดประโยชน์ต่อมารดามากเมื่อเทียบกับความเสี่ยง
ของผลข้างเคียงที่อาจเกิดต่อทารกในครรภ์หรือทารกแรก
คลอด
