Developing simulation model for fetal position assessment training skill

Abstract

Practicing pregnancy examination and delivery by nursing student requires knowledge and understanding about fetal position. The current practice model is unable to conveniently rotate the fetus and the fetus may not be in the required position. The purpose of this research and development (R&D) was to design and develop a model for training in fetal position assessment, evaluate quality and effectiveness of the model. The study was divided into three parts; 1) design and development process of the model. 2) evaluating quality of the model. and 3) evaluating the effectiveness of the model. The sample were sophomore nursing students, registered for Maternal Newborn Nursing and Midwifery Practice 1 in summer semester of academic year 2016. Simple random sampling and sampling without replacement was used. The instruments were quality evaluation of model form which showed the content validity index of 1. The evaluation form showed the reliability of Cronbach’s alpha efficiency of 0.87 and fetal position assessment form showed the reliability of KR-20 of 0.80. Data were analyzed using descriptive statistics and comparison of pre and post-fetal position assessment scores using paired t-test.  

             Results of the study showed that; 1) the model consisted of two pieces; one being a pelvic and fetus model mounted on a steel base. Pelvic model is made from Mache-paper according to size of the pelvic and fetus model is equipped with a sprocket which can rotate left and right. And the second piece is QR code video which describes the fetal position of the vertex presentation. 2) The quality of the model has the highest average score (  = 4.78, S.D. = 0.42) and 3) fetal position assessment score after training with model was increased statistically significant (t = 18.20, p < 0.001).

            The result of this study benefited nursing students in fetal position assessment training before their practicum in antenatal care unit and delivery department. The model was appropriate for students to practice and manually review fetal positions, encourage to improve fetal position assessment skill.
