การพัฒนาระบบสุขภาพชุมชนโดยชุมชนเพื่อชุมชน: กรณีศึกษาตำบลหนองหาร อำเภอสันทราย จังหวัดเชียงใหม่

บทคัดย่อ
การพัฒนาระบบสุขภาพชุมชนโดยชุมชนเพื่อชุมชน มีวัตถุประสงค์เพื่อศึกษาศักยภาพชุมชน และการพัฒนาโครงการและดำเนินการตามปัญหาที่ชุมชนเลือก แบบมีส่วนร่วมของประชาชน องค์กรปกครองส่วนท้องถิ่น สถานบริการสุขภาพ และสถาบันการศึกษา กลุ่มตัวอย่างในการศึกษาแบ่งเป็น 3 กลุ่มคือ บุคลากรองค์กรปกครองส่วนท้องถิ่น สถานบริการสุขภาพ และตัวแทนชุมชน จำนวน 56 คน ดำเนินการศึกษาโดยการสัมภาษณ์ การสัมภาษณ์เชิงลึก การสนทนากลุ่ม การประชุมเชิงปฏิบัติการ และการจัดเวทีเสวนา เพื่อการแลกเปลี่ยนเรียนรู้ จากนั้นนำข้อมูลไปสะท้อนคิดเป็นระยะๆ ร่วมกับประชาชนในชุมชนและหน่วยงานที่เกี่ยวข้อง เพื่อหาแนวทางในการสร้างเสริมและแก้ไขปัญหาสุขภาพ จากนั้นนำข้อมูลมาวิเคราะห์เชิงเนื้อหา จัดหมวดหมู่และสรุปผล ผลการศึกษาพบว่า ชุมชนมีทุนทางสังคม มีศักยภาพ และระบบการจัดการปัญหาสุขภาพในเรื่องต่างๆ โดยใช้ชุมชนเข้ามามีส่วนร่วมตั้งแต่การค้นหาปัญหาชุมชน และร่วมดำเนินการในระบบการจัดการปัญหาสุขภาพของชุมชน ทั้งนี้ชุมชนได้เลือกปัญหาที่ต้องการแก้ไขคือกลุ่มเยาวชนเสี่ยงต่อการติดสารเสพติด จึงจัดโครงการพัฒนาแกนนำเยาวชนสู่ชุมชนต้นแบบห่างไกลยาเสพติดขึ้น ซึ่งชุมชนมีแผนจะพัฒนาต่อยอดเป็นแหล่งเรียนรู้และสร้างเครือข่ายการทำงานของเยาวชนในพื้นที่อื่นต่อไป

Abstract
The purpose of community health systems development was to studypotential of the community and develop and implement a project to solve communityproblem according to by itself and using people participatory strategies by involvedLocal Administrative Organizations, health service sectors, and educational institutions.The samples were divided into 3 groups: Local Administrative Organizations, healthservice sectors and the community member representatives all together 56 persons.Data were collected by interviews, in-depth interviews, groups discussion, workshop, andpeople’s forum to exchange and share knowledge among study groups. The collecteddata were reflected periodically to community members and participating sectors to findsolution to community problems and to promote community health. Then, data wereanalyzed using content analysis, categorized, and summarized. It was found that thecommunity has social capital, potential, and a system that use community participationin the management of community health problems. Community members involvedin the process of community problems identification, and implementation of communityhealth problem management system. In this study, the community has selected issuesof youth to resolve. Youth are at risk of drug addiction, so the community initiates“The project of youth leadership development to be a community model against drug abuse”.The community plans to further develop as a learning center and to create a networkof youth to further working in other areas.
