Abstract

Most deaths occur among people who are old and experiencing multiple chronic illnesses. Older people are the vulnerable group who have been left out from the palliative care system since this system was originally designed to meet the needs of cancer patients. Not only cancer, but other chronic diseases such as chronic obstructive pulmonary disease, end stage chronic kidney disease and heart failure were also major causes of death among older people. Moreover, older people are experiencing multiple diseases and geriatric syndromes such as dementia caused by degenerative changes and being barriers for health care providers to assess palliative care needs and decide for services. To achieve the ultimate goal of quality of life at the end of life, palliative care system should be urgently developed to ensure that older people suffering from multiple chronic diseases will receive continuity of care throughout the course of illness.
