Factors Related to Shared Decision-Making Needs Regarding Treatment of Patients’ Families in a Medical Intensive Care Unit
Keywords:

Patients admitted to the medical intensive care unit (MICU) include severely ill persons with acute multiple organ failure and uncertain progression. Most of them are unable to make decisions by themselves, meaning that families have to take part in decision-making. The aim of this descriptive-correlational study was to examine factors related to shared decision-making needs for treatment in the MICU. Eighty-five families of critically ill persons were recruited from the MICU, Maharaj Nakorn Chiangmai Hospital using a simple random sampling technique. Research instruments were composed of four parts, 1) the family’s information, 2) needs of family regarding the medical treatment questionnaire, 3) Acute Physiology and Chronic Health Evaluation II (APACHE II), and 4) Consumer Quality Index Relative in Intensive Care Unit. Descriptive and Pearson’s correlational analysis were used to analyze data.
The results showed that:
1. The shared decision-making needs regarding medical treatment of critically ill patients’ families were at a high level (94.11%).
2. Severity of illness in medical care units was at a high level (77.59%).
3. Quality of care, according to family’ s perceptions, was at a good level (91.76%).
4. The shared decision-making needs regarding medical treatment of critically ill patients’ families had a small positive significant relationship with severity of illness (r = .023, p < .05).
5. The shared decision-making needs regarding medical treatment of critically ill patients’ families had a small positive significant relationship with quality of care according to families’ perceptions (r = .120, p < .05).
The results of this research provide foundational information to promote quality of care so that critically ill patients’ families can share decision-making needs regarding medical treatment.
