ปัจจัยที่เกี่ยวข้องกับความต้องการการมีส่วนร่วมตัดสินใจในการรักษา ของครอบครัวผู้ป่วยในหอผู้ป่วยหนักอายุรกรรม

ผู้ป่วยหนักอายุรกรรม เป็นผู้ที่มีความเจ็บป่วยรุนแรงจากการสูญเสียหน้าที่ของอวัยวะสำคัญอย่างเฉียบพลันหลายอวัยวะ มีการดำเนินโรคที่ไม่แน่นอน ผู้ป่วยหนักส่วนใหญ่ไม่สามารถตัดสินใจในการรักษาด้วยตนเองได้ ดังนั้นครอบครัวต้องมีส่วนร่วมตัดสินใจในการรักษา การวิจัยเชิงพรรณนาแบบหาความสัมพันธ์ครั้งนี้ มีวัตถุประสงค์เพื่อหาปัจจัยที่เกี่ยวข้องกับความต้องการการมีส่วนร่วมตัดสินใจในการรักษาของครอบครัวผู้ป่วยหนักอายุรกรรม กลุ่มตัวอย่างคือ ครอบครัวผู้ป่วยที่เข้ารับการรักษาในหอผู้ป่วยหนักอายุรกรรม โรงพยาบาลมหาราชนครเชียงใหม่ จำนวน 85 ราย เลือกกลุ่มตัวอย่างแบบสุ่มอย่างง่าย เครื่องมือวิจัยประกอบด้วย 4 ส่วน คือ 1) แบบสอบถามข้อมูลส่วนบุคคลของครอบครัวผู้ป่วย 2) แบบสอบถามการมีส่วนร่วมตัดสินใจในการรักษาของครอบครัว 3) แบบประเมินความเจ็บป่วยอย่างเฉียบพลันและภาวะสุขภาพเรื้อรังฉบับที่ 2 และ 4) แบบประเมินคุณภาพการดูแลผู้ป่วยตามการรับรู้ของครอบครัว วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ Pearson’s correlational analysis
ผลการวิจัยพบว่า
1. ความต้องการการมีส่วนร่วมตัดสินใจในการรักษาของครอบครัวผู้ป่วยในหอผู้ป่วยหนักอายุรกรรมอยู่ในระดับสูงถึงร้อยละ 94.11
2. ความรุนแรงของการเจ็บป่วยของผู้ป่วยในหอผู้ป่วยหนักอายุรกรรมอยู่ในระดับมากถึง ร้อยละ 77.59
3. คุณภาพการดูแลผู้ป่วยตามการรับรู้ของครอบครัวอยู่ในระดับดีร้อยละ 91.76
4. ความต้องการการมีส่วนร่วมตัดสินใจในการรักษาของครอบครัวมีความสัมพันธ์ทางบวกในระดับต่ำกับความรุนแรงของการเจ็บป่วยของผู้ป่วยในหอผู้ป่วยหนักอายุรกรรมอย่างมี นัยสำคัญทางสถิติ (r = .023, p < .05)
5. ความต้องการการมีส่วนร่วมตัดสินใจในการรักษาของครอบครัวมีความสัมพันธ์ทางบวกในระดับต่ำกับคุณภาพการดูแลผู้ป่วยตามการรับรู้ของครอบครัวอย่างมีนัยสำคัญทางสถิติ (r = .120, p < .05)
ผลการศึกษาครั้งนี้ นำเสนอข้อมูลพื้นฐานในการพัฒนาคุณภาพการดูแล ดังนั้นครอบครัวผู้ป่วยหนักสามารถมีส่วนร่วมตัดสินใจในการรักษา
