ปัจจัยที่เกี่ยวข้องกับความพร้อมในการจำหน่ายจากโรงพยาบาล ในผู้ป่วยสูงอายุที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ

เมื่อผู้ป่วยสูงอายุที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจออกจากโรงพยาบาล จึงต้องมีความจำเป็นในการเตรียมความพร้อมแก่ผู้ป่วย ครอบครัว และผู้ดูแลเพื่อให้ผู้ป่วยกลับไปพักฟื้นที่บ้านอย่างปลอดภัย ลดการเกิดปัญหาและภาวะแทรกซ้อนที่อาจเกิดขึ้นได้ การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่เกี่ยวข้องกับความพร้อมในการจำหน่ายจากโรงพยาบาลในผู้ป่วยสูงอายุที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจได้แก่ อายุ เพศ รายได้ ลักษณะการอยู่อาศัย ประสบการณ์การรักษาด้วยวิธีผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจ ระยะเวลาในการนอนโรงพยาบาล คุณภาพการสอนก่อนจำหน่าย และการประสานการดูแลก่อนจำหน่าย และศึกษาความสัมพันธ์ระหว่างปัจจัยดังกล่าวกับความพร้อมในการจำหน่ายจากโรงพยาบาล คัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง จากผู้ป่วยสูงอายุที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจที่แพทย์อนุญาตให้จำหน่ายออกจากโรงพยาบาลมหาราชนครเชียงใหม่ จำนวน 52 ราย รวบรวมข้อมูลโดยการใช้ แบบสอบถามความพร้อมในการจำหน่ายจากโรงพยาบาล แบบสอบถามคุณภาพการสอนก่อนจำหน่าย และแบบสอบถามการประสานการดูแลก่อนจำหน่ายจากโรงพยาบาล ซึ่งนำไปทดสอบความเชื่อมั่นโดยใช้สูตรสัมประสิทธิ์อัลฟาของครอนบาค ได้ค่าความเชื่อมั่นเท่ากับ .80, .84 และ .83 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติสัมประสิทธิ์สหสัมพันธ์แบบพอยท์ไบซีเรียล และสถิติสหสัมพันธ์แบบเพียร์สัน
ผลการศึกษาวิจัย พบว่า
- กลุ่มตัวอย่างมีคะแนนความพร้อมในการจำหน่ายจากโรงพยาบาลในระดับสูง
- กลุ่มตัวอย่างส่วนใหญ่เป็นเพศชาย (ร้อยละ 69.23) มีอายุระหว่าง 60-69 ปี (ร้อยละ 71.16) มีรายได้เฉลี่ยต่อเดือนต่ำกว่า 5,000 บาท (ร้อยละ 50) อาศัยอยู่กับคู่สมรสและบุตรหลาน (ร้อยละ 48.08) กลุ่มตัวอย่างทั้งหมดไม่เคยมีประสบการณ์การเข้ารับการรักษาด้วยวิธีการผ่าตัดทำทางเบี่ยงหลอดเลือด
หัวใจ และมีระยะเวลาในการนอนโรงพยาบาลเฉลี่ย 11.81 วัน กลุ่มตัวอย่างให้คะแนนคุณภาพการสอนก่อนจำหน่าย และคะแนนการประสานการดูแลก่อนจำหน่ายอยู่ในระดับสูง
- ปัจจัยด้านการบำบัดทางการพยาบาล ได้แก่ คุณภาพการสอนก่อนจำหน่าย และการประสานการดูแลก่อนจำหน่ายมีความสัมพันธ์ทางบวกกับความพร้อมในการจำหน่ายจากโรงพยาบาล (r= .524, p < .01; r= .432, p < .01 ตามลำดับ) สำหรับปัจจัยส่วนบุคคลและปัจจัยด้านการเข้ารับการรักษาในโรงพยาบาลไม่มีความสัมพันธ์กับความพร้อมในการจำหน่ายจากโรงพยาบาล
ผลการศึกษาวิจัยครั้งนี้ได้ข้อมูลพื้นฐานสำหรับพยาบาลและบุคลากรทางด้านสุขภาพในการวางแผนการจำหน่าย เพื่อเตรียมความพร้อมในการจำหน่ายจากโรงพยาบาลแก่ผู้ป่วยสูงอายุที่ได้รับการผ่าตัดทำทางเบี่ยงหลอดเลือดหัวใจได้อย่างเหมาะสมต่อไป
