ผลของการพยาบาลระบบสนับสนุนและให้ความรู้ต่อพฤติกรรมการดูแลตนเอง และการควบคุมน้ำหนักตัวในผู้ที่ได้รับการคาสายสวนหลอดเลือดดำ ชนิดระยะยาวสำหรับการฟอกเลือดด้วยเครื่องไตเทียม

ผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการคาสายสวนหลอดเลือดดำชนิดระยะยาว ต้องได้รับการสนับสนุนและให้ความรู้เพื่อทำให้สามารถดูแลตนเองได้ การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของการพยาบาลระบบสนับสนุนและให้ความรู้ต่อพฤติกรรมการดูแลตนเองและการควบคุมน้ำหนักตัว กลุ่มตัวอย่างเป็นผู้ป่วยโรคไตเรื้อรังระยะสุดท้าย ที่ได้รับการคาสายสวนหลอดเลือดดำชนิดระยะยาวและได้รับการฟอกเลือดด้วยเครื่องไตเทียม ในโรงพยาบาลเอกชนแห่งหนึ่งในจังหวัดพิษณุโลก จำนวน 60 ราย โดยแบ่งเป็นกลุ่มทดลอง และกลุ่มควบคุมกลุ่มละ 30 ราย กลุ่มทดลองได้รับการพยาบาลระบบสนับสนุนและให้ความรู้ กลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือการวิจัยประกอบด้วย แบบสอบถามข้อมูลทั่วไป แผนการพยาบาลระบบสนับสนุนและให้ความรู้ที่พัฒนาจากแนวคิดของโอเร็ม สื่อนำเสนอภาพนิ่ง คู่มือการดูแลตนเอง แบบประเมินพฤติกรรมการดูแลตนเอง เครื่องชั่งน้ำหนักระบบอิเล็กทรอนิกส์หน่วยเป็นกิโลกรัม และบันทึกน้ำหนักตัว วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติ chi-square test, Fisher's exact test, paired t-test และ independent t-test.
ผลการศึกษาพบว่า คะแนนพฤติกรรมการดูแลตนเองของกลุ่มทดลองภายหลังได้รับการพยาบาลระบบสนับสนุนและให้ความรู้ (M = 65.73, SD = 3.66) สูงกว่าก่อนได้รับการพยาบาลระบบสนับสนุนและให้ความรู้ (M = 42.33, SD = 1.68) อย่างมีนัยสำคัญทางสถิติ (p < 0.001) และสูงกว่ากลุ่มควบคุม (M = 45.83, SD = 2.56) อย่างมีนัยสำคัญทางสถิติ (p < 0.001) และหลังการทดลองพบว่า กลุ่มทดลองจำนวน 22 คน คิดเป็นร้อยละ 73.3 และกลุ่มควบคุมจำนวน 20 คน คิดเป็นร้อยละ 66.7 คุมน้ำหนักตัวได้ตามเกณฑ์ แต่พบว่าไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ
การพยาบาลระบบสนับสนุนและให้ความรู้ สามารถส่งเสริมพฤติกรรมสุขภาพที่ดีขึ้น ในผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการคาสายสวนหลอดเลือดดำชนิดระยะยาวสำหรับการฟอกเลือดด้วยเครื่องไตเทียม ควรมีการติดตามผลลัพธ์ระยะยาวเพื่อให้เห็นประสิทธิภาพชัดเจนขึ้น
