บทคัดย่อ

ภาวะวิกฤตระบบประสาท เป็นภาวะที่เกิดขึ้นอย่างเฉียบพลันและรุนแรง ส่งผลให้ผู้ป่วยมีปัญหาเกี่ยวกับระดับความรู้สึกตัว การเคลื่อนไหว และการสื่อสาร เนื่องจากความรุนแรงของโรคทำให้ผู้ป่วยต้องได้รับการรักษาพยาบาลอย่างใกล้ชิดในหอผู้ป่วยวิกฤต นอกจากนี้ ครอบครัวของผู้ป่วยไม่สามารถแสดงบทบาทดูแลผู้ป่วยและเกิดความเครียดร่วมด้วย การวิจัยกึ่งทดลองครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการดูแลแบบบูรณาการของครอบครัวต่อการมีส่วนร่วมในการดูแล และความเครียดของครอบครัว กลุ่มตัวอย่างคัดเลือกแบบเฉพาะเจาะจงตามเกณฑ์การคัดเลือก เป็นครอบครัวผู้ป่วยวิกฤตระบบประสาทที่เข้ารับการรับการรักษาในหอผู้ป่วยหนัก โรงพยาบาลลำพูน จำนวน 50 คน แบ่งเป็นกลุ่มควบคุม และกลุ่มทดลองกลุ่มละ 25 คน โดยจับคู่ให้มีลักษณะใกล้เคียงกัน กลุ่มควบคุมได้รับการดูแลตามปกติ ส่วนกลุ่มทดลองได้รับการดูแลตามโปรแกรมการดูแลแบบมีส่วนร่วมของครอบครัว เครื่องมือที่ใช้ในการวิจัยแบ่งเป็น 2 ส่วน ได้แก่ 1) เครื่องมือในการดำเนินงาน ประกอบด้วยโปรแกรมการดูแลแบบบูรณาการของครอบครัวและคู่มือการดูแล และ 2) เครื่องมือเก็บรวบรวมข้อมูล ประกอบด้วย แบบบันทึกข้อมูลทั่วไปของผู้ป่วยและครอบครัว แบบประเมินการมีส่วนร่วมในการดูแล และแบบประเมินความเครียดของครอบครัว วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติ independent t-test และ paired t-test

การศึกษาพบว่า ครอบครัวผู้ป่วยวิกฤตระบบประสาทกลุ่มทดลองมีคะแนนเฉลี่ยการมีส่วนร่วมในการดูแล (M = 63.24, SD = 0.43) สูงกว่ากลุ่มควบคุม (M = 29.40, SD = 4.56) อย่างมีนัยสำคัญทางสถิติ (t(48) = 62.73, p < .001) และมีคะแนนเฉลี่ยความเครียด (M = 41.60, SD = 5.91) น้อยกว่ากลุ่มควบคุม (M = 75.16, SD = 7.38) อย่างมีนัยสำคัญทางสถิติ (t(48) = 5.91, p < .001) นอกจากนี้ การลดลงของความเครียดของครอบครัวกลุ่มทดลอง (M = 43.08, SD = 4.41) มากกว่ากลุ่มควบคุม (M = 10.40, SD = 7.87) อย่างมีนัยสำคัญทางสถิติ (t(48) = 18.09, p < .001)

การศึกษานี้ แสดงให้เห็นถึงประสิทธิผลของโปรแกรมการดูแลแบบบูรณาการของครอบครัวที่ช่วยเพิ่มการมีส่วนร่วมของครอบครัวในการดูแลผู้ป่วยวิกฤตระบบประสาท และลดความเครียดของครอบครัวได้
