ปัจจัยทำนายพฤติกรรมสุขภาพของผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราว หลังได้รับการวินิจฉัยจากแพทย์ 6 เดือนขึ้นไป

โรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราว ถือเป็นสัญญาณเตือนสำคัญของการเกิดโรคหลอดเลือดสมองในอนาคต การส่งเสริมพฤติกรรมสุขภาพที่เหมาะสมและต่อเนื่องในผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราว จึงมีความจำเป็นอย่างยิ่งในการป้องกันโรคหลอดเลือดสมองซ้ำ การวิจัยเชิงพยากรณ์แบบมีความสัมพันธ์นี้ มีวัตถุประสงค์เพื่อศึกษาตัวทำนายทางจิตวิทยาที่สำคัญของพฤติกรรมสุขภาพในกลุ่มบุคคลที่ได้รับการวินิจฉัยว่าเป็นโรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราวมาแล้วอย่างน้อย 6 เดือน โดยอ้างอิงกรอบแนวคิดความเชื่อด้านสุขภาพของเบคเกอร์ กลุ่มตัวอย่างเป็นผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราวที่มารับการรักษาแบบผู้ป่วยนอกจำนวน 160 ราย จากโรงพยาบาล 3 แห่งในประเทศไทย โดยใช้การสุ่มแบบโควต้าเพื่อให้ได้สัดส่วนที่เหมาะสม เครื่องมือที่ใช้ในการเก็บข้อมูล ได้แก่ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบสอบถามพฤติกรรมสุขภาพในผู้ที่โรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราว 3) แบบสอบถามการรับรู้โอกาสเสี่ยงต่อการเกิดโรคหลอดเลือดสมอง 4) แบบสอบถามการรับรู้ความรุนแรงของโรคหลอดเลือดสมอง 5) แบบสอบถามการรับรู้ประโยชน์ของการปฏิบัติพฤติกรรมสุขภาพ และ 6) แบบสอบถามการรับรู้อุปสรรคของการปฏิบัติพฤติกรรมสุขภาพ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติ stepwise multiple regression analysis
ผลการวิจัยพบว่า มีเพียงการรับรู้อุปสรรคของการปฏิบัติพฤติกรรมสุขภาพเท่านั้น ที่สามารถทำนายพฤติกรรมสุขภาพได้อย่างมีนัยสำคัญทางสถิติ (ß = –.431, p < .001) โดยสามารถอธิบายความแปรปรวนของพฤติกรรมสุขภาพได้ร้อยละ 18.1 (Adjusted R² = .181, F = 36.057, p < .001) ขณะที่องค์ประกอบอื่นในกรอบแนวคิดความเชื่อด้านสุขภาพ ได้แก่ การรับรู้โอกาสเสี่ยง ความรุนแรง และการรับรู้ประโยชน์ ไม่เป็นตัวทำนายที่มีนัยสำคัญ
ผลการศึกษานี้ สะท้อนให้เห็นถึงความจำเป็นที่บุคลากรทางการพยาบาลควรพัฒนาและดำเนินกิจกรรมการพยาบาลที่มุ่งลดอุปสรรคของการปฏิบัติพฤติกรรมสุขภาพในผู้ที่เป็นโรคหลอดเลือดสมองขาดเลือดมาเลี้ยงชั่วคราว หลังได้รับการวินิจฉัยจากแพทย์ 6 เดือนขึ้นไป พยาบาลมีบทบาทสำคัญในการสนับสนุนผู้ป่วยให้มีพฤติกรรมสุขภาพที่ดีและป้องกันการเกิดโรคหลอดเลือดสมองซ้ำ
