คุณภาพชีวิตของเด็กวัยเรียนที่ป่วยด้วยโรคเรื้อรัง: การวิจัยผสานวิธีแบบคู่ขนาน

คุณภาพชีวิตในเด็กวัยเรียนที่เจ็บป่วยด้วยโรคเรื้อรัง จำเป็นต้องได้รับการดูแลแบบองค์รวมจากครอบครัว โรงเรียน และบุคลากรทีมสุขภาพ การวิจัยเชิงพรรณนาครั้งนี้ ใช้ระเบียบวิธีการวิจัยผสานวิธีแบบคู่ขนาน มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิตของเด็กวัยเรียนที่ป่วยด้วยโรคเรื้อรัง กลุ่มตัวอย่างเป็นเด็กวัยเรียนที่มารับบริการที่หน่วยตรวจผู้ป่วยนอกหรือเข้ารับการรักษาที่หอผู้ป่วยกุมารเวชกรรมในโรงพยาบาลแห่งหนึ่ง ในระยะที่ 1 เป็นการวิจัยเชิงปริมาณทำการสำรวจในเด็กวัยเรียนจำนวน 184 คน เลือกกลุ่มตัวอย่างโดยใช้วิธีแบบสะดวกตามคุณสมบัติที่กำหนด เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามคุณภาพชีวิตสำหรับเด็ก (PedsQLTM 4.0) และแบบสอบถามข้อมูลส่วนบุคคล วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนา และ chi-squared test ระยะที่ 2 เป็นการวิจัยเชิงคุณภาพ โดยใช้แนวคำถามแบบกึ่งมีโครงสร้างในการสัมภาษณ์เชิงลึก กลุ่มตัวอย่าง จำนวน 16 คน คัดเลือกกลุ่มตัวอย่างแบบเจาะจง วิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เชิงเนื้อหา
ผลการวิจัยเชิงปริมาณพบว่า คุณภาพชีวิตของเด็กวัยเรียนที่เจ็บป่วยด้วยโรคเรื้อรังในภาพรวมอยู่ในระดับที่ดี โดยร้อยละ 57.6 เป็นเด็กที่มีคุณภาพชีวิตอยู่ในระดับปกติ และร้อยละ 42.4 มีคุณภาพชีวิตระดับต่ำ หากจำแนกเป็นรายด้านพบว่า คุณภาพชีวิตด้านร่างกาย ด้านอารมณ์ และด้านสังคม อยู่ในระดับปกติ ส่วนคุณภาพชีวิตด้านโรงเรียนอยู่ในระดับต่ำ เมื่อวิเคราะห์เปรียบเทียบโรคที่เจ็บป่วยและระดับคุณภาพชีวิตพบว่า คุณภาพชีวิตของผู้ป่วยเด็กวัยเรียนในแต่ละโรคไม่มีความแตกต่างกัน และผลการวิจัยเชิงคุณภาพพบว่า คุณภาพชีวิตของเด็กวัยเรียนที่เจ็บป่วยด้วยโรคเรื้อรัง มี 3 ประเด็นหลัก คือ 1) ใช้ชีวิตปกติภายใต้ข้อจำกัด: มุมมองจากเด็กที่มีคุณภาพชีวิตดี 2) ชีวิตที่ไม่เหมือนคนอื่น: ภาพสะท้อนของเด็กที่มีคุณภาพชีวิตต่ำ และ 3) ภาระที่ต้องจัดการสุขภาพ
ผลการศึกษานี้ ทำให้เข้าใจคุณภาพชีวิตของเด็กวัยเรียนที่ป่วยด้วยโรคเรื้อรังมากขึ้น สามารถใช้เป็นข้อมูลพื้นฐานในการจัดรูปแบบการช่วยเหลือเด็ก โดยเฉพาะด้านการเรียน และการกำหนดกลยุทธ์เพื่อสนับสนุนการปรับตัวต่อการเจ็บป่วย
